We are a fully merged PCN over three sites based in Fareham, Whiteley and Titchfield.

We are offering an exciting opportunity for the right individual to join our growing multi-disciplinary clinicians. We are currently looking to expand and require an Advanced Nurse Practitioner/Advanced Paramedic Pratitioner to deliver same day care to individuals. This will include telephone triage, history taking, clinical assessment, diagnosis, treatment, and evaluation of care. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills. You will demonstrate critical thinking in the clinical decision-making process

This vacancy is working part time 30 hours per week on a Monday, Tuesday and Wednesday.

Main duties of the job

Act as Advanced Nurse Practitioner/Advanced Paramedic Practitoner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.

Work as an autonomous practitioner to undertake consultations, using own clinical judgement to diagnose, treat or refer patients and provide clinical case management for same day access.

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.

Ensure compete and accurate documentation of each and every patient contact.

Make direct referrals to primary, secondary, and social services within locally agreed pathways, guidance and protocols.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Job Description

As an Advanced Nurse Practitioner (ANP)-Advanced Paramedic Partitioner you will be an experienced clinician who, acting within your professional boundaries, will provide same day care for patients registered at Meon Health Practice. This will include telephone triage, initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills. You will demonstrate critical thinking in the clinical decision-making process.

KEY RESPONSIBILITIES

Clinical Care

Act as Advanced Nurse Practitioner-Advanced Paramedic Practitioner demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing/paramedic roles.
