An exciting
opportunity has arisen for the right individual to join our growing
multi-disciplinary clinicians. We are
currently looking to expand and require an Advanced Nurse Practitioner to
deliver same day care to individuals as well as looking after patients in their
homes or visiting residential and nursing homes. This will include telephone triage, history
taking, clinical assessment, diagnosis, treatment, and evaluation of care. You
will demonstrate safe, clinical decision making and expert care, including
assessment and diagnostic skills. You
will demonstrate critical thinking in the clinical decision-making process.

Main duties of the job
Act as Advanced Nurse Practitioner, demonstrating advanced
clinical competence and a knowledge base beyond those associated with
conventional nursing roles.
Work as an autonomous practitioner to undertake consultations,
using own clinical judgement to diagnose, treat or refer patients and provide
clinical case management for same day access, housebound/care home patients
with acute/chronic/palliative care needs and those at risk of deterioration in
health that may result in unplanned, avoidable hospital admission or necessary length
of hospital stay.
Diagnose and manage both acute and chronic conditions, integrating
both drug and non-drug-based treatment methods into a management plan.
Ensure compete and accurate documentation of each and every
patient contact.
Make direct referrals to primary, secondary, and social services
within locally agreed pathways, guidance and protocols. Ensure appropriate DNACPR decisions are in
place.
Support patients to adopt health promotion strategies that promote
healthy lifestyles and apply principles of self-care.
You will undertake proactive
consultations with Nursing Residential home and housebound patients within
the Meon Health Practice. You will provide advice to care home staff on
patient care and wider health policy. This includes patient specific care plans
and DNACPR decisions.

development (CPD) opportunities to ensure that up to date evidence-based
knowledge and competence in all aspects of the role is maintained including
maintaining up to date CPR training and other core competencies.

Record accurate, timely consultation data
within the patients records.

Work collaboratively with colleagues within and external to the
Practice.

Pro-actively promote the complex care role within the Meon Health Practice
and externally to key stakeholders and agencies.

remain up to date and improve service delivery.
Non-clinical team role
The post-holder will:

Identify appropriate opportunities to delegate both clinical and
administrative tasks to other appropriate staff.

Help the practice operate in a cost-effective manner.

Identify and manage care risks on a continuing basis.

Participate in practice meetings as required.

Participate in audits and inspections as appropriate.

Work closely with the doctors and managers in
the setting up and/or improving of Meon Health Practice systems for monitoring/measuring performance
against clinical governance and quality targets.

The post-holder is required to travel
independently between sites, patient homes and to attend meetings etc hosted by
other agencies .The post-holder will have contact with bodily fluids, i.e.
wound exudates, urine, blood etc whilst in clinical practice and must take
precautions as required to minimise the risk of infection/contamination using
PPE.
