Job summary

As a Diabetes Nurse you will provide specialist support, screening services, health education and advice to patients. You will act autonomously within your professional scope of practice providing care for patients both face to face and by telephone consultation. Working alongside the Diabetes Team you will implement improvements to ensure the service is responsive to the Patients needs.

Main duties of the job

You will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigation in the appropriate manner.You will be integral to the Multidisciplinary Team acting as patient advocate and providing expert knowledge and guidance. You will also provide supervision and training to peers and junior staff.

Job responsibilities

KEY/MAIN RESPONSIBILITIES

  • Participate in the administrative and professional responsibilities of the Diabetes Team
  • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
  • Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Organisation.
  • Attend and participate in Organisations meetings as required Provide Expert Clinical Practice Applying critical reasoning and professional judgement to nursing issues and decisions.
  • Identifying patients that are of clinical risk and taking appropriate action to ensure a safe environment for the patients, families and staff.
  • Developing and maintaining relationships with external providers that support clinical and patient needs.
  • Ensuring patients are actively reviewed on time utilising practice recall systems.
  • Ensuring maximum utilisation of clinical time.
  • Utilising advanced clinical assessment skills to identify potential or actual problems and collaborating with the clinical team to proactively address these
  • Managing incoming referrals appropriately, ensuring optimal patient outcomes are achieved
  • Assisting in the review, redesign, documentation and implementation of care pathways for patients with diabetes
  • Using advanced nursing knowledge and skills to provide comprehensive health assessment, diagnosis, intervention and treatment services that meet nursing practice standards and optimise patient outcomes
  • Provides specialty advice to support and manage patients physical and psychological needs
  • Ensure you are aware of appropriate policies and procedures relating to diabetic patients
  • Ensuring care is patient and family centered and planned with regard to individual cultural needs
  • Working efficiently as a team member providing quality care and ongoing support to patients, for example:
  • Counselling and initiating support services
  • Collection of specimens
  • Blood pressure monitoring
  • Communicating test results to patients
  • Provide health promotion and education to patients
  • Providing general and specific health assessments to individuals, families and groups with an emphasis on wellness. For example, at risk individuals, chronic disease management - diabetes
  • Providing on-going education to individuals, families and groups. Teach skills for health maintenance and prevention of accidents and illness. This may include:
  • Health and lifestyle education e.g. nutrition, smoking cessation, alcohol & exercise
  • Teaching of patient self-monitoring procedures
  • Diabetic reviews
