Job summary

An exciting opportunity has arisen for the right individual to join a growing team of multi-disciplinary clinicians delivering care to patients within Meon Health Practice. We are currently looking to expand our Complex Care Team who deliver care to both individuals in their own home and patients in residential and nursing homes.

This vacancy is working 2 days a week.

Main duties of the job

Remote consulting and visiting Meon Health Practice patients in CQC registered Care homes (nursing / residential) and housebound patients registered with the network practices to achieve acute visiting and assessment early in the day (typically before mid-day) to optimise patient outcomes.

The role will comprise two specific disciplines:

1. Working within the Meon Health Practice complex care Team you will undertake proactive consultations with Nursing / Residential home and housebound patients within the Meon Health Practice. You will provide advice to care home staff on patient care and wider health policy. This includes patient specific care plans and DNACPR decisions.

2. Working alongside a centrally based duty GP you will deliver a high standard of patient care to patients who have been placed onto a central GP triaged visiting list by practices within Meon Health Practice.

Job description

Job responsibilities

As an Advanced Practitioner within the Meon Health Practice Complex care team, you will be using advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base, to provide patient care. This will include referring patients to hospital or other community services. You will be working autonomously (in the patients home or a care home) with access to electronic patient records and available escalation to the complex care GP or duty GP for advice and support as required. You will work alongside Clinical Pharmacists from within Meon Health Practice and the ICB to deliver the appropriate medicines related care

Alongside the Meon Health Practice complex care Team, provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems & social health needs. Using skills in history taking, physical examination, problem-solving and clinical decision-making, you will establish a diagnosis and management plan

Provide assessment, treatment and diagnosis at point of first contact, by attending to patients in a variety of clinical or non-clinical settings according to patients needs

Undertake home, nursing and residential care home visits, in accordance with the relevant protocols

Make professionally autonomous decisions for which you will be accountable

Refer patients to an alternative care setting or treat & discharge as appropriate

Instigate necessary invasive/non-invasive diagnostic tests/investigations and interpret findings/reports.

Ensure that professional standards are maintained and within the guidance issued by the DoH (Department of Health), the HCPC (Health and Care Professions Council) and the NMC

Work with multi-disciplinary teams within the SHN and healthcare community to promote integrated and seamless pathways of care

Contribute to Meon Health Practice achieving its quality and quantitative targets to sustain the high standards of patient care and service delivery

Participate in the assessment of community health needs and help to develop patient and family-centred strategies to address them

Help to develop and set up new patient services and participate in initiatives to improve existing patient services

Assist with development of patient care plans and ensure appropriate DNACPR decisions are in place.

Use Associated Clinical Groups informatics to identify patients needing additional care or at risk of hospital admission.

Assess frailty use Rockwood frailty scales/FI

The post-holder will:

Promote evidence-based practice via latest research-based guidelines and the development of practice-based research

Monitor the effectiveness of their own clinical practice through quality assurance strategies such as the use of audit and peer review

Maintain any professional registration

Participate in continuing professional development (CPD) opportunities to ensure that up to date evidence-based knowledge and competence in all aspects of the role is maintained including maintaining up to date CPR training and other core competencies

Record accurate, timely consultation data within the patients records

Work collaboratively with colleagues within and external to the Practice

Pro-actively promote the complex care role within the Meon Health Practice and externally to key stakeholders and agencies

Identify own learning needs in order to remain up to date and improve service delivery
