Job summary
This is a new ACP-Paramedic role which gives the opportunity to the successful candidate to work across 4 practices, for our PCN Enhanced access and Hub service. There is plenty of flexibility about which days or hours you work. Our core hours are Monday-Friday 8am-8pm and Saturday 8am-5pm. The successful candidates timetable can be flexibly organised around their needs (childcare, portfolio etc) in consideration with the needs of the population.

Main duties of the job
The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced paramedic who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The paramedic ACP in primary and urgent care will adhere to legal, regulatory and ethical requirements, professional codes, and employer protocols.
Specifically, this means working in accordance with the Health & Care Professions Council (HCPC) Standards of conduct, performance and ethics and; Standards of proficiency: Paramedics.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Clinical practice
The paramedic ACP has the following key responsibilities:
They will work as part of a multi-disciplinary team (MDT) within the PCN.
They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary or secondary care team.
They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.
They will be able to:
operform specialist health checks and reviews within their scope of practice and in line with local and national guidance
operform and interpret ECGs
operform investigatory procedures as required, and
oundertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance.
They will support the delivery of anticipatory care plans and lead certain services (e.g. monitoring blood pressure and diabetes risk of elderly patients).
They will provide an alternative model to urgent and same day GP home visit for the network.
They will communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
They will maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices
Support PCN and practice work around QOF, NIS and DES requirements.
