Job summary

An exciting opportunity has arisen for a motivated and caring Advanced Nurse Practitioner/Advanced Clinical Paramedic to join our friendly, high achieving and supportive group of practices.

Main duties of the job

The post holder will be working as part of the wider Elmham Group of Practices Teams which include North Elmham and Swanton Morley Practices.

With support from the General Practitioners and existing Advanced Nurse Practitioners and Advanced Clinical Paramedic, the post holder will be responsible for providing care of the presenting Patient through initial history taking, clinical assessment, investigations leading to appropriate diagnosis and treatment pathways. Demonstrate safe, clinical decision-making and acting within Professional boundaries expected. To work collaboratively as part of the Multi-Disciplinary Team. Providing support to colleagues within their direct team. Assisting with the development of Protocols, Education, and training programmes, along with involvement in Research and Audit Programmes where appropriate.

About us

Elmham Group of Practices (EGoP) is a GP Surgery in rural Mid-Norfolk serving a population of 10,000 patients across two sites in Elmham and Swanton Morley. Our main site is set in beautiful surrounding countryside in North Elmham with the other site within comfortable proximity. The fine City of Norwich is easily reached by car within 35 minutes.

EGoP has a team of 40+ health care professionals and administration support staff. Both sites offer a welcoming and friendly environment to work in and team spirit amongst surgery staff is high. The 5 Partners, Management Team, Clinical Team, and Administration Team communicate well between them.

Our fantastic multi-skilled Clinical Team consists of General Practitioners, Nurse Practitioners, Practice Nurses, and Health Care Assistants to carry out all the necessary duties to deliver a comprehensive General Practice service ensuring the highest standard of care to all our patients. We are part of the PCN for Mid Norfolk and through our network we have a range of ARRS roles within our practice to support us in delivering high-quality care to our patients. We have an on-site Dispensary. We are proud to be a training practice to GP Registrars.

The post holder will be working as part of the wider team serving the surgeries at North Elmham and Swanton Morley. With support from the Partners and other Nurse Practitioners, the post holder will provide care for the presenting patient from the initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making ensuring that they always act within their professional boundaries. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients. The post holder will provide support to the Clinical Manager on clinical audit, research, the development of practice clinical guidelines, procedures and protocols, education programmes and quality initiatives such as QOF and CQC (Care Quality Commission) inspections.

Person Specification

The successful candidate will be:

- NMC (Nursing and Midwifery Council) (Nursing and Midwifery Council) Registered with current and up to date Revalidation and Registration.

- Qualified Nurse Practitioner with an appropriate level Degree/Masters.

- Hold a valid Non-Medical Prescriber qualification.

- Hold a clean and valid Driving Licence.

- Have experience with Minor and Acute presenting illness with advanced level decision making skills.

- Hold the ability to work autonomously and have good decision-making skills.

- Primary Care experience is preferred but not essential.

- Knowledge of working with System 1.

- Knowledge of working to QOF (Quality Outcomes Framework) requirements.

- Ability to follow protocol and procedural practice and National guidelines

- Excellent team working and communication skills

- Flexible and multiskilled with practice based and home visiting patient care and assessment.

DBS

The post would be subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and the provision of a satisfactory DBS check.
