Job summary

This is an on-site role as a Clinical Pharmacist at The Sides Medical Centre located in Swinton. position hours required are 37 hours per week.

The Pharmacist would need to be able to provide a comprehensive pharmacy service to the patients of the practice. The pharmacist's primary responsibility will be to ensure that patients receive appropriate medicine and other treatments by providing pharmaceutical care.

You will play a key role in optimizing medication therapy and ensuring safe and effective use of medications.

The pharmacist role will be required to work as part of a multidisciplinary team and collaborate with other healthcare professionals to improve patient outcomes.

Main duties of the job

Provide support and guidance to clinical staff and pharmacy technicians employed in the practice and contribute to training other healthcare professionals in the practice on how to work in general practice.

Review patients medication management, undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

This role requires experienced pharmacist with proven experience of working within General Practice and is an experienced prescriber.

The candidate will need to be capable of provide clinical leadership regarding patient centred medicines optimisation and quality improvement.

The Sides Medical Centre delivers a high standard of care and contributes in our local Swinton PCN. The Sides is a large practice with 13,500 patients with 10 doctors, a pharmacy teams and a nursing team.

They are all supported by our highly trained administrator staff and management team, who will support you in your role.

The candidate will need to be motivated and committed to providing high standard of quality patient care, while ensuring practice policies and procedures are followed.

Competing Medication reviews - taking responsibility for the care and management of patients where medicine optimisation is required. E.g., COPD, asthma.

Patient facing clinical medication reviews - undertake clinical medication reviews with patients and make appropriate recommendations for the clinical team on prescribing and monitoring.

Through structured medical reviews the post holder will support patients to take their medications to get the best from them, reduce waste and promote self-care.

Signposting to community pharmacy and referring to GPs or other healthcare professionals as appropriate.

Management of repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.

Provide specialist expertise in the use of medicines whilst addressing both the public health and social care needs of patients and also tackling health inequalities.

Develop relationships and work closely with other pharmacy professionals across Swinton PCN and the wider health and social care system. Qualifications

Essential

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years experience as a pharmacist, demonstrated within a Practice portfolio.
  • Hold an independent prescribing qualification.
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in General Practice.
  • Recognises priorities when problem solving and identifies deviations from normal pattern and can refer as appropriate.
  • Able to follow legal, ethical, professional, and organisational policies and procedures and codes of conduct.
  • Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
  • Understands and demonstrates the characteristics of a role model to members in the team and/or service
  • Demonstrates understanding of the mentorship process
  • Demonstrates ability to conduct teaching and assessment effectively according to a learning plan with supervision from a more experienced colleague
