Job summary

An exciting role for an Advanced Nurse Practitioner to be part of one of the most innovative Medical Practice teams in West Yorkshire. You shall be part of a team of a large multi-professional team of GPs, Advanced Nurse Practitioners, Practice Nurses, Pharmacists and HCAs. Our ANPs lead on specific areas of interest. We would value applicants with special interests in Palliative or Cancer Care.

Ths successful candidate shall deal with 2 clinical sessions of just over 15 patients each - AM and PM with a mid-day 3 hour mid-day period to review bloods , under take a Nursing Home or Acute visit.

A full working day is approximately 9 hours, and there may be a late PM session, on one day of the week. A full working week usually is 4 days, but we can be flexible.

You will be an independent prescriber, fully registered and licensed with the NMC as an RGN with a right to work in the UK. You shall hold a record of good standing as a Nursing colleague with the NMC and a clean driving licence.

The post holder is expected to be up to date with current regional and national prescribing guidance, as well as NICE guidance.

Main duties of the job

As an ANP colleague , you shall be expected to see an average of 30 F2F or Telephone patient contacts per day.

Fair share of pathology to review, action and file.

Whilst we do have 3 Clinical Pharmacists, all prescribing clinicians may stil receive complex medication queries.

The ANP will need to work effectively and work to similar timescales as GPs do (12 mins appointments) and be able to undertake onward referrals into secondary care and also undertake all relevant investigations and manage theirinterpretation. Home visits where required. Care and Nursing Home Weekly Ward rounds where required.

You will independently assess, take history and diagnose a range of acute and chronic presentations as part of a multi-disciplinary team. The practice offers the ability to develop as a Clinical Lead in an egalitarian environment where all clinicians have an equitable voice. All decisions in relation to the practice are discussed at weekly Clinician and Senior Management Team meetings and implemented. The practice is a 2 site facility with the branch surgery being closed temporarily.

The Grange is a respected, long standing, well regarded large GP practice in Huddersfield, serving c.16,500 patients in Fartown, Birkby, Deighton and surrounding areas. The main site is 5 minutes from the M62 and easily accessible from all of West Yorkshire. The practice has successfully implemented and overhaul of teams and processes over the last 4 years.

The post holder needs to understand that whilst the core work is General Practice, after the first few years, this is an opportunity to develop into areas and opportunities that is beyond the realms of normal General Practice that the practice and wider Practice team are involved in delivering. In terms of remuneration, we have a sliding scale for the postholder that is amongst the best available scales for ANP colleagues.

For the successful candidate, the above scale should not be a bar in ensuring that they get paid the very best scales accessible in the region, but this is always commisurate with experience as an ANP.
