Job summary

Orchard Surgery Dereham are looking to recruit an experienced Nurse Practitioner/ Emergency Care Practitioner to work a minimum of three days.

Main duties of the job

The role will be based at the practice having clinics and visiting patients in their own homes within the locality, if required.

You will have a Nurse Practitioner or Emergency Care Practitioner qualification and a prescriber is desirable but not essential.

The Successful candidate should be able to demonstrate high standards of clinical care, sound decision making and excellent interpersonal skills.

Salary is negotiable depending upon qualifications and experience

We are a thriving, busy practice in the centre of Dereham with currently 11300 patients and increasing numbers. We are in the process of a full refurbishment to gain additional clinical rooms and looking for an enthusiastic person to join our team.

Job Summary

The Nurse Practitioner/Emergency Practitioner will attend to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness; seeing and advising people in respect of their continuing medical and nursing needs. These patients will have booked appointments at 15-minute intervals. There is a requirement to see extra or emergency patients.

The nurse practitioner will need to prioritise and triage the needs of patients both face to face and on the telephone, making any necessary referrals or investigations in the appropriate manner.

Key Responsibilities

Clinical Practice

The following are the core responsibilities of the nurse practitioner/emergency care practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a.Provide a triage service, actively managing or referring patients or treating patients within own scope of practice

b. Support the clinical team in the development of individual treatment plans for chronic disease patients

c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Request pathology services as necessary, processing and interpreting results as required

f. Refer patients directly to other services as appropriate

g. Delivering patient care as necessary, referring patients to secondary / specialist care as required

h. Maintain accurate clinical records in conjunction with extant legislation

i. Ensure read codes are used effectively

j. Chaperone patients where necessary

k. Prioritise health issues and intervene appropriately

l. Support the team in dealing with clinical emergencies

m. Recognise, assess and refer patients presenting with mental health needs

n. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

o. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

p. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

q. Support the clinical team with all safeguarding matters, in accordance with local and national policies

r. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

s. Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the nurse practitioner/emergency care practitioner may be requested to:

a.If an independent prescriber, prescribe in a safe an effective manner, in accordance with PGD and extant legislation

b. Act as the audit lead, effectively utilising the audit cycle

c. Monitor and ensure the safe storage, rotation and disposal of medicaments

d. Contribute to the implementation of an effective training programme for staff and students within the practice

e. Provide clinical supervision for the nursing team

f. Participate in local initiatives to enhance service delivery and patient care

g. Support and participate in shared learning within the practice

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives

j. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA).

k. Maintain professional registration and adhere to revalidation requirements
