Orchard Surgery is looking to recruit an Experienced Nurse Practitioner / Visiting Nurse to work four days per week as part of our multi-disciplinary team completing clinics and home visits with the aim to reduced avoidable admissions to hospital
Main duties of the job
The successful candidate will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for our patients within the Surgery, their own homes and care homes.
The nurse practitioner / visiting nurse will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The post-holder will use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment.
The successful candidate must own their own vehicle as home visits will be an expectation

Orchard Surgery is a purpose built Surgery in the centre of Dereham Town.
We currently have 11,200 patients registered with the Surgery and care for four residential homes.
We are part of Mid-Norfolk Primary Care Network and Dr Melanie Hill (Partner) is our Clinical Director.

- Work as part of the clinical team, to provide consultations and home visits for patients presenting with a variety of conditions making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.
- To undertake home visits in accordance with the relevant protocols
- To make professional autonomous decisions for which he/she is accountable for
- To refer patients to an alternative care setting or treat and discharge as appropriate
- To work collaboratively with the multi-disciplinary team and the wider integrated care team, to meet the needs of the patients and help avoid any unnecessary admissions to hospital.
