An exciting opportunity has arisen for a suitably qualified Practice Nurse to join our dynamic and innovative team . We are an EMIS Web forward thinking practice, in search of an enthusiastic and committed Practice Nurse to join our friendly team. As a Practice Nurse, you will be an essential part of our well established nursing team, undertaking various responsibilities such as chronic disease management, health promotion, and clinical procedures. Main duties of the job
Deliver practice nursing
services within NMC Code of Conduct and agreed Practice standards.
Manage patients with chronic
diseases, following quality frameworks and evidence-based guidelines.
Contribute to the development of
integrated services within Primary Care.
Perform clinical duties,
including chronic disease management, cervical screening, immunisations, and
lifestyle advice.
Recognise signs of abuse and
work with safeguarding teams where relevant.
Maintain accurate records,
participate in audits, and update disease registers.
About us
Pendleside Medical Practice is a friendly, well-established and respected GP Practice based at Clitheroe Health Centre in the centre of Clitheroe. We are a teaching and dispensing practice rated outstanding by CQC at our last inspection and consistently high achievers in QoF.
We
have 8 GP Partners and salaried GPs supported by a multidisciplinary nursing
team, ACPs, clinical pharmacists and skilled practice administration and
reception teams who serve a patient list size of over 11700 for the population of
Clitheroe and the surrounding areas of the Ribble Valley.
We form part of Ribblesdale Primary Care Network and have excellent working relationships with the other 3 neighbouring member practices supporting the 42,500 population of the network.

To work within NMC Code of Conduct and to agreed Practice standards and protocols.
To deliver effective implementation of management of patients with Chronic Diseases as detailed by the Quality and Outcomes framework as well as both local and national quality schemes.
Supporting the implementation of evidence based guidelines (NICE) in conjunction with the wider practice team.
Participate and engage in the practice's appraisal process working towards a personal development plan using a reflective approach to practice.
Actively participate in multi-disciplinary practice and clinical meetings.
Help develop and support practice clinical p rotocols.
Provide Chronic disease management in COPD, Asthma, CHD/CVD and Diabetes.
Provide both adult and child immunisations.
Help ensure safe storage, rotation and disposal of vaccines and drugs.
Supporting patients to engage in a healthier lifestyle with evidence based advice and signposting to appropriate support.
Provide high quality evidence based care.
Maintain and update disease registers.
Promote and support the practice's learning culture by assisting in clinical instruction, mentoring and supervision of trainees and less experienced staff members to the practice.
