Job summary
To deliver evidence-based practice for patients presenting with long-term conditions, and the provision of preventative health care to the practice population. You will work with the practice team to meet the needs of the patients.
Main duties of the job
The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population which will include :
- Management of long term conditions
- Deliver appropriate treatment, screening and advice within the professional competence of the post-holder to the practice population
- Develop and manage care programmes for individual patients within specific groups
- Support and deliver Cervical Screening
- Deliver immunisation programmes
- Health Promotion
- Participate in Quality Outcome Framework (QOF) and Extended Primary Care Service objectives
- To work within the NMC Code of Conduct
