Job summary

We are a lovely practice based in Harefield in West London looking for a P/T or F/T permanent Nurse Practitioner to join our team.

The practice has approx. 9,000 patients and has 3 GP Partners and a great clinical and non clinical team supporting.

We have regular clinical team meetings to discuss any cases that individuals may wish to discuss or that team members can learn from.

We are a training practice and have regular GP's training with us within the practice.

Main duties of the job

Duties include:

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factor and early signs of illness

Engaging with activities such as QOF, LES/DES and other additional work essential for providing a full service for patients

In consultation with patients and in line with current practice disease management protocols, developing care plans for health

Prescribing in accordance with the practice prescribing formulary and the CCG whenever this is clinically appropriate

Clinically examine and assess patient needs and plan clinical care accordingly.

Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary

Utilise sensitive communication styles to ensure patients are fully informed and consent to treatment

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Deliver care according to NSF, NICE guidelines and evidence-based care.

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.

Support and participate in shared learning across the practice and wider organisation.

LEADERSHIP AND DEVELOPMENT

Take responsibility for own learning and performance acting as a positive role model.

Assist in planning and implementing changes within the area of care and responsibility.

Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.

Contribute to the development of local guidelines, protocols and standards.

Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (eg courses and conferences).

Ensure appropriate supervision of safe storage, rotation and disposal of vaccines and drugs. Oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements where appropriate.

MANAGING INFORMATION

Use technology as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.

Collate, analyse and present clinical data and information to the team.

CRITICAL REQUIREMENTS

Ability to work with minimal supervision after initial training period.

An understanding, acceptance and adherence to the need for strict confidentiality of both patient records and business activities of the partnership.
