Job summary

We are seeking a driven and enthusiastic Advanced Clinical Practitioner with experience in advanced nursing to join our team. The ideal candidate will be capable of working autonomously and safely within their scope of practice, while also collaborating effectively with our multidisciplinary team and external colleagues. Main duties of the job

The role involves delivering high-quality clinical care through the assessment, diagnosis, and management of patients with acute and complex conditions in both general practice and care home settings. Key responsibilities include conducting face-to-face and telephone consultations, developing management plans, prescribing medications, and ensuring continuity of care through regular reviews. The practitioner will support palliative and anticipatory care planning, participate in care home visits, and collaborate with the wider healthcare team to provide seamless, patient-centered services. Strong communication skills are essential for engaging patients, carers, and colleagues effectively, while maintaining accurate records and adhering to legal, clinical, and practice standards. The role also emphasises teamwork, continuous professional development, patient safety, and contribution to service improvements. Upholding confidentiality, equality, and health and safety protocols are integral, along with managing challenging situations in line with practice policies.

Key Responsibilities:

  • Provide high-quality clinical assessment, diagnosis, and management of patients presenting with minor and potentially serious acute conditions in general practice and care home settings.
  • Work alongside duty GPs and other ACPs as part of the duty team to deliver timely, evidence-based care.
  • Conduct assessments via telephone triage and face-to-face appointments, prioritising patient needs effectively.
  • Take comprehensive patient histories, perform physical examinations, and identify red flag symptoms.
  • Develop differential diagnoses and formulate appropriate management plans, including arranging investigations and analysing results.
  • Prescribe and review medications in line with national and practice protocols, ensuring therapeutic effectiveness.
