Job summary

We are seeking an enthusiastic and compassionate Community Nurse / Practice Nurse to join our Health Inclusion Team (Integrated Neighbourhood Team).

This rewarding role focuses on delivering high-quality, person-centred care to marginalised and underserved populations, including people who may experience homelessness, social exclusion, or barriers to accessing healthcare.

As part of our multi-disciplinary team, you'll provide holistic nursing care in the community setting, primarily homeless hostels and patients' homes. You will bring sound community/primary care nursing experience, including essential confidence in wound care management, alongside a passion for supporting vulnerable individuals to improve their health and wellbeing.

Main duties of the job

Key Responsibilities:

Provide high-quality, holistic nursing care within community and practice settings.

Undertake assessments, develop care plans, and review patient needs.

Deliver wound care assessment and management, ensuring evidenced-based practice.

Work collaboratively within the Health Inclusion Team (INT).

Promote self-care and health education amongst patients.

Participate in outreach and engagement activities to improve access to care for marginalised groups.
