We are seeking a motivated and detail-oriented Pharmacy Technician to join our busy GP practice team. You will play a vital role in supporting the safe and effective delivery of medicines management, patient care, and clinical services within the practice.
Salary Band 4/5 depending on experience
Main duties of the job

- Medication Reconciliation using clinical letters and post Hospital Discharge Summaries
- Medicine Management including prescription screening and processing
- Respond and communicate to patient related medication queries
- Communicate with local pharmacies regarding stock supplies issues and shortages
- Medication synchronisation in line with regular repeat prescription
- Medication monitoring recalls (e.g BP, blood tests, annual reviews)
- Prescribing audits in line with CQC/QOF targets as set by the Prescribing Team (training can be provided)
- High Risk Drug monitoring,
- Standard/Annual medication monitoring
- ICB Medicine Incentive Schemes
- Shared Care Protocols
- Action medicines safety alerts from CAS/MHRA/ICB
- Level 1 and 2 Medication reviews (training can be given)
- Assist with Pre-SMR (Structure Medication Review) Screening (training can be given)
