Job summary

Due to the expansion of the practice, The Lakes Medical Centre is looking to recruit a Nurse Practitioner or an Advanced Nurse Practitioner to join our friendly and supportive team based in the Salford area. We have 6 strong GP team which also includes Physician associate, 3 Practice nurses, Practice Pharmacist and Health Care Assistant who work closely to deliver care to our 10,000+ patient population.

Main duties of the job

To act as an autonomous prescribing practitioner working independently and in conjunction with other health care professionals to assess, diagnose, and treat patients within a primary care setting.

About us

Our premises also house Community mental health and Social prescribing clinics. We practise from purpose-built premises with easy access to Manchester city centre and the North West motorway network. As an established training practice, we are proud to have trained Advanced Nurse Practitioners, foundation-year doctors and GPST trainees.

We use EMIS web, Docman and Accurix to deliver care to our patients

Job responsibilities

The Nurse Practitioner/Advanced Nurse Practitioner will be responsible for:

* Face to Face Patient Care in 15-minute appointments

* Prescribing safe, effective medication appropriately in line with best guidance

* Assessing patients, ordering and interpreting investigations, diagnosing and treating

* Ensuring clinical standards meet all Care Quality Commission and Service Level requirements

* Directly admitting patients with acute medical needs to secondary care

* Referring patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for X-ray, Ultrasound, blood tests and Microbiology.

The successful candidate will be able to demonstrate:

* Experience working as an Advanced Nurse Practitioner in primary care

* High levels of motivation and ability to work autonomously

* Excellent written and verbal communication

* Excellent people management skills with the confidence to work with patients with varying clinical conditions

The successful candidate will have a named GP mentor within the practice to ensure personal development aims are met and help you grow in your role.
