Job summary

25 hours a week = 5 hours per day Monday - Friday

We are looking for a Practice Pharmacy Technician to provide technical services to our GP/ Clinical Team, professional advice and direct support.

You will liaise with patients, clinical and non-clinical staff within the Practice team, but also develop MDT work with hospitals, community pharmacies and patients' Carers with the aim of improving the pharmaceutical care of our patients.

As a small team we are able to move quickly to adhere to new guidelines and we are supportive of green policies and only appropriate prescribing of controlled drugs for our patients.

We utilise NHS provide IT technology using digitalisation to improve our service. As a team we are and will continue to embrace technology for service improvements.

Main duties of the job

  • act as a Technical Pharmacy Advisor to Practice Clinicians
  • advise Practice Management and other non-clinical staff concerning administrative aspects of medicines management
  • manage repeat prescribing requests, resolving queries, producing prescriptions for clinical review
  • align prescriptions, review dates, work on discharge summaries, coordinate community team requests, support Care Homes work, coordinate medical device requests
  • work with GP team to support Care Home work
  • provide independent information to patients over their medications and prescribing changes.

About us

We are a 5200+ patient practice, and growing quickly, based in the heart of Derby city with two GP Partners. We have a predominately young patient list with approximately 70% under 45 years old. The majority of our incoming patient contact is virtual via our newly updated website. We have a modernised phone system and aim to continue to embrace technology for the benefit of our patients. Our patient contacts are high and our wait times are low, based on a national footprint.

We have a new, small PPG but aim to bring in new innovations to gain better and more diverse feedback from our patients. 1. JOB SUMMARY

To provide specialist pharmaceutical technical services, professional advice and direct support to Practice GPs, Nurses, Pharmacist, HCAs and Reception/Administrative Staff

To develop the role of the Practice Pharmacy Technician to maximise support to all Clinical and Non-clinical practice staff to improve patient care in relation to all aspects of medicines and their impact

To provide professional specialist medicines advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists, Community Nurses and other health professionals to promote the safe, effective and high quality, cost effective use of medicines in all prescribing decisions for the benefit of the Practice's patients

2. MAJOR DUTIES AND RESPONSIBILITIES

To act as a Technical Pharmacy Advisor to Practice Clinicians

To advise Practice Management and other non-clinical staff concerning administrative aspects of medicines management as appropriate

To manage repeat prescription requests, resolving queries where possible and producing prescriptions for clinical review and signature

To address the following areas without the need to consult a GP/Clinical Pharmacist in each case (although clinical support and advice will always be available if required):

Review dates following medication reviews

Facilitate and document correct addition & removal of medications following receipt of a discharge summaries, secondary care and community clinic letters.

Tidying repeat medication lists including synchronising durations and removing medications not ordered within a defined period

Changing acute medications to repeat where appropriate- e.g. A new medication that a GP has commenced and there is subsequently evidence for the need for the item on a repeat basis and necessary tests/checks have confirmed this

Addition/Removal of Medication for Care Home patients under GP guidance

Co-ordinate District Nurse Medication requests for the practice population

Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds and stoma care products.

To work in conjunction with the nominated Clinicians who support our Care Homes

Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate

To review practice stock management and develop systems to improve processes.

Analyse Practice prescription data to produce reports as requested

Participate in review and setting of policies relating to medicines management and prescribing

Implement initiatives to reduce waste of prescribed medicines

Support and influence the Practice Clinical, Management and Administrative Teams to implement procedural and service changes relating to prescribing to ensure best practice is adopted in accordance with local guidelines, legislation, directives and safety updates e.g. NICE, MRHA etc.

Contribute to the annual Meds Management audit and review

3. SUPERVISION RECEIVED

The postholder will be operationally line managed by the Practice Manager with clinical supervision provided by Partners.

4. SUPERVISION EXERCISED

The postholder has no day to day line management responsibility and does not directly manage any members of staff. However they are responsible for mentoring and supervising non-clinical staff to support our work
