Exciting Opportunity: Paramedic Deal & Sandwich PCN

We are looking for an enthusiastic and experienced Paramedic to join our dedicated team across Deal and Sandwich Primary Care Network (PCN).

In this patient-facing, multi-disciplinary role, you will provide high-quality care through assessment, diagnosis, treatment, and appropriate referral. Your responsibilities will also include telephone triage, participation in a weekly MDT meeting, and involvement in care home ward rounds.

Youll play a key role in delivering the Enhanced Health in Care Homes (EHCH) service, supporting residents across 26 care/nursing homes within the PCN. This service aims to deliver personalised, proactive care in collaboration with care home staff, ultimately improving outcomes and quality of life for residents.

Main duties of the job

The Paramedic will play a key role in our Enhanced Health in Care Homes (EHCH) Team, working with Paramedics and HCA/Care Coordinators, as well as the wider PCN team, including Social Prescribers, Clinical Pharmacists, Pharmacy Technician, Mental Health Practitioners, and Care Coordinators.

Working within professional boundaries, the postholder will support improved patient outcomes and the efficiency of primary care by:

  • Triaging, assessing, diagnosing, and managing patients with appropriate care plans

  • Ensuring continuity of care through follow-ups and reviews

  • Delivering EHCH DES requirements, including care home ward rounds and liaising with GP practices

  • Participating in weekly MDT meetings, working with services such as KCHFT, Frailty Team, Mental Health, and Social Care

  • Liaising with external agencies to ensure vulnerable patients and care homes are supported

  • Identifying and signposting patients at risk of long-term conditions

  • Accurately recording patient interactions using EMIS and relevant templates

The Paramedic will be a key member of our Enhanced Health in Care Homes (EHCH) Team, which includes fellow Paramedics and HCA/Care Coordinators, while also contributing to the wider PCN team comprising Social Prescribing Link Workers, Clinical Pharmacists, a Pharmacy Technician, Mental Health Practitioners, and Care Coordinators.

Working within professional boundaries, the successful candidate will help enhance patient outcomes, support continuity of care, and improve service delivery across care homes in the PCN area.

  • Undertake clinical reviews for non-urgent concerns, general health checks, and personalised care/support planning.

  • For new care home residents and hospital discharges, complete an initial review within 7 working days including full assessment, treatment escalation planning, personalised care planning, and DNACPR discussion (where appropriate).

  • Use sound clinical judgment, prioritise caseloads, and refer for investigations as required.

  • Develop and maintain care plans in collaboration with patients, families, and the MDT.

  • Support care coordination and make timely referrals to community and specialist services.

  • Communicate complex or sensitive medical information effectively, including discussing end-of-life care options with patients and families.

  • Provide non-urgent and acute clinical input during weekly care home ward rounds.

Essential

  • BSc in a training programme approved by the College of Paramedics
  • Active and relevant professional registration with HCPC
  • Completed two-year Consolidation of Learning period as a 'newly qualified paramedic' AND have a further three years' experience as a band 6 (or equivalent) paramedic
  • Commit to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of the commencement of the role.

Desirable

  • Evidence of verification against the paramedic primary care roadmap
