Job responsibilities

The Paramedic will be a key member of our Enhanced Health in Care Homes (EHCH) Team, which includes fellow Paramedics and HCA/Care Coordinators, while also contributing to the wider PCN team comprising Social Prescribing Link Workers, Clinical Pharmacists, a Pharmacy Technician, Mental Health Practitioners, and Care Coordinators.

Working within professional boundaries, the successful candidate will help enhance patient outcomes, support continuity of care, and improve service delivery across care homes in the PCN area.

• Undertake clinical reviews for non-urgent concerns, general health checks, and personalised care/support planning.

• For new care home residents and hospital discharges, complete an initial review within 7 working days including full assessment, treatment escalation planning, personalised care planning, and DNACPR discussion (where appropriate).

• Use sound clinical judgment, prioritise caseloads, and refer for investigations as required.

• Develop and maintain care plans in collaboration with patients, families, and the MDT.

• Support care coordination and make timely referrals to community and specialist services.

• Communicate complex or sensitive medical information effectively, including discussing end-of-life care options with patients and families.

• Provide non-urgent and acute clinical input during weekly care home ward rounds.

• Evaluate treatment plans and offer advice to care home staff.

• Contribute to staff education to enhance delivery of safe, effective care.

• Build effective working relationships across health, social care, and voluntary sectors.

• Liaise with the wider PCN team, community services, SECAmb, KCHFT, frailty teams, local hospitals, and social care.

• Signpost or refer patients appropriately to support holistic care delivery.

• Use sensitive, person-centred communication styles to ensure patient understanding and consent.

• Adapt communication methods to meet individual needs, including cultural and cognitive differences.

• Prioritise and manage your workload independently while promoting high-quality care.

• Participate in shared learning and quality improvement initiatives within the PCN and wider organisation.

• Effectively use EMIS and other clinical systems to plan care, record interventions, and support audit processes.

• Maintain professional registration and engage in ongoing CPD and supervision.

• Support a learning culture by sharing knowledge and contributing to team development.

• Participate in training, reflective practice, and relevant courses or conferences.

• Promote equality and diversity in all interactions.

Person Specification

Qualifications

Essential

• BSc in a training programme approved by the College of Paramedics
• Active and relevant professional registration with HCPC
• Completed two-year Consolidation of Learning period as a 'newly qualified paramedic' AND have a further three years' experience as a band 6 (or equivalent) paramedic
• Commit to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of the commencement of the role.

Desirable

• Evidence of verification against the paramedic primary care roadmap

Experience

Essential

• Experience in managing the care of patients presenting with undifferentiated undiagnosed conditions and- or a designated caseload, using theoretical and practical experience, completing examinations and assessments, care planning, implementing interventions, referring appropriately, and evaluating outcomes aligned to care plans
• Experience of working within a multi-disciplinary team
• Experience of working under own direction in a patient focused environment
• Uses skills in a range of routine situations requiring analysis or comparison of a range of options

Desirable

• Experience of working within a primary care environment
• Evidence of working across organisational boundaries within health and social care
• Experience of EMIS

Knowledge and Skills

Essential

• Understands and acts in line with HCPC professional standards for practice
• Ability to write comprehensive, accurate clinical notes, implement and evaluate care plans
• Good IT skills including ability to record accurate clinical notes
• Ability to communicate effectively - written, verbal and non-verbal communication- with patients, relatives , carers and all members of the multi-disciplinary team
• Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
• Understanding of Confidentiality and the Data protection Act
• Excellent interpersonal, influencing and negotiating skills
• Able to work under pressure and to meet deadlines

Desirable

• Knowledge of primary care networks and collaborative working
