Job description

Job responsibilities

1. To work with the GPs from The Medical Group to deliver co-ordinated care within the community and reduce the need for hospital admission.
2. To lead the collaboration with The Medical Group to ensure best outcomes for their registered patients
3. To facilitate patients to stay in their own home as long as possible.
4. To keep patients, their families and carers fully involved and informed during the episode of care.
5. To liaise and co-ordinate care with multiple agencies (where appropriate) to ensure the patient needs are met.

Person Specification

Qualifications

Essential

• * First level registered nurse
• * Relevant first level degree
• * Relevant postgraduate study
• * Significant recent post registration clinical experience
• * Independent prescriber
• * Experience and in depth knowledge of long term condition
• management and the complex needs of patients and their carers in a
• community setting
• * Experience of working autonomously
• * Case management skills
