Administrator/Clinical Coder reporting to Practice Manager

Permanent. The administrative element of the role requires organisational and IT/data entry skills and attention to detail. Familiarity with the EMIS Web computer system and Docman would be advantageous. The coding element is to review the clinical paperwork workflow (patient discharge summaries, consultant clinic letters etc.). The purpose of the role is to ensure that GPs only see items of correspondence they need to action, and the rest is on the patients record ready for their next interaction and appropriately coded. This involves following a protocol, so that the process is safe and effective. Strong skills at picking out key information from a range of clinical correspondence are essential. Training will be made available to the successful candidate.
To thrive in this key role, you will preferably have excellent admin skills and desirable clinical knowledge and/or experience of clinical coding. You will read hospital correspondence for a large portion of the day as well as undertaking data entry and coding, and will need to understand the implications of what you are coding, and when to file or forward letters on to other staff. You will proactively follow-up issues you identify where possible (e.g. ordering a further blood test), seeking appropriate support and clearly documenting what you have done.
You will need to develop a broad understanding of General Practice, and an in-depth knowledge of medical terminology, coding and the clinical coding process.
Key Tasks & Responsibilities
Input patient information into computer.
Convert relevant diagnostic and procedural details into alpha-numeric codes.
Enter coded information accurately and consistently.
Review and interpret the content of hospital and other clinical letters, discriminating between letters requiring action and those which do not, and identifying relevant coding and action points.
Workflow of tasks back to GPs or other healthcare professionals that require further action, advice or input.
The administrator part of the role includes but not limited to: Recalling patients who need long term conditions, immunisations and other investigations or tests. Searches run at defined intervals, checks made and actioned accordingly. Statistics show that work has been undertaken and patients have been booked for reviews and follow-ups overall % of reviews achieved across the year. Demonstrates good understanding of EMIS Web, including relevant searches and documents required. Scanning documentation. Locating and filing medical records. Dealing with subject access requests including photocopying and/or sending documentation and invoicing where required. Providing support to research projects in which the Practice is participating. Other data entry or coding tasks. Summarise patient notes (paper and electronic) onto the clinical computer system for newly registered patients and for those already registered with us.
Work as a member of a multi-professional team, sharing clinical/administrative experience and working towards a patient-centred approach provision.
With other members of the Practice and Network Teams, support the delivery of the local APMS contracted work, including the Quality & Outcomes Framework.
Attend multi-disciplinary meetings and education sessions organized by the Practice when appropriate.
Actively engage in assessment and development of competence through observation and supervised practice.
Keep up to date with relevant medical research and evidence-based medical practice.
Regularly reflect on own practice (and keep a record of learning encounters) in order to identify learning needs and encourage self-directed lifelong learning and continued professional development.
Practice within professional code of conduct and recognise limitations accordingly.
To work safely and abide by Practice policies and procedures including Infection Control, Information Governance, Health & Safety and Manual Handling.
To work according to child protection and adult protection guidelines at all times.
To work with the Practice Manager to develop an annual personal/professional development plan, undertaking training relevant to the post.
To observe complete confidentiality at all times and to ensure that there are no breaches of confidentiality as a result of your actions.
To carry out any other duties as may reasonably be requested by the Practice Manager and the partners.
