Job summary

You will be working as part of a primary care team that prides itself on the delivery of high-quality healthcare services and you will be well supported by GPs as well as our friendly and enthusiastic nursing and admin team. The post is up to 37.5 hours per week and hours are negotiable. We have high levels of patient satisfaction and good access.

There will always be a fully qualified GP on site to provide clinical support should the need arise.

Main duties of the job

To provide face to face minor illness management, working as part of a multi-disciplinary team providing urgent, on the day (Same Day Urgent Care) to the registered patients of Brierley Park Medical Group.

To provide highly skilled evidence-based nursing to patients as an essential part of the health care provided by the practice.

To support the development and delivery of policy and procedures and providing clinical leadership to appropriate others as required.

The role requires you to function autonomously and within your sphere of competency whilst being assisted and supervised by GPs. We also have a strong team of ARRS staff including pharmacists, pharmacy technicians, first contact physiotherapists, mental health, social prescribers.

About us

Due to retirement Brierley Park Medical Group is looking to welcome an experienced minor illness nurse to join our friendly clinical team to support the delivery of excellent care for our patients. Experience of independent working in primary care is essential; an interest in diabetes is desirable.

We are a 5 partner GMS practice using SystmOne, with a list size of 18,500 patients. Currently have 6 GP trainees as well as nursing students. We are a level 4 research practice.

Brierley Park Medical Group is one of 4 practices in the Ashfield North PCN.

Job description

Job responsibilities

Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness; seeing and advising people in respect of their continuing medical and nursing needs. These patients will have booked appointments at 15 minuteintervals. There is a requirement to see extra or emergency patients. The Minor Illness nurse will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. Patients with problems needing referral to secondary care should be discussed with the duty GP before making such referral.

DUTIES AND RSPONSIBILITIES

The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction GP Partners and will be dependent on current and evolving practice need.

Minor Illness Management:

To demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, working within their scope of practice.

To participate in assessing, diagnosing and treating patients presenting with minor illness, including giving advice on and prescribing medicines (where this is within the scope of practice, competence and confidence)

To discharge patients or refer them as appropriate to other healthcare professionals / external agencies

To provide appropriate care and services to patients in all age ranges and level of health care needs, according to their level of competency and code of conduct

To work closely with the wider urgent care teams in delivering high quality patient centred clinical care

To support the triage and streaming process, including contributing to the triage and streaming decision making when required

To participate in assessing patients with more complex presentations waiting to see the streaming clinician (GP or Advanced Clinical Practitioner)

To work in partnership with patients and their carers to assess individual health needs, plan appropriate care and use every appropriate opportunity to promote health and reduce the incidence or exacerbation of disease

To be capable and confident in digital consultation where this is required

To work as an autonomous practitioner within appropriate clinical and professional guidelines.
