Job summary

An exciting opportunity has arisen for an enthusiastic motivated pharmacist to join our innovative, award-winning pharmacy team. Based across South Kerrier PCN, our patient population of just over 33, 000 patients spread across five practices: Helston Medical Centre, Mullion & Constantine Group, Cober Valley Health, St Keverne Health Centre and Isles of Scilly St Marys Health Centre. Our pharmacy team are highly valued, well integrated and respected amongst our PCN and individual practices.

The team is well established with a senior lead clinical pharmacist, a senior audit/systems pharmacist, clinical pharmacist & three pharmacy technicians . With a strong training ethos, as a team we are involved in training both undergraduate and graduate pharmacy staff as well as the wider MDT. We have a rolling audit programme which has been acknowledged at local and national level and are well recognised for our success in improving patient safety. We are a friendly, supportive, and welcoming team and very much value our members and wholly support their development. We hold regular team meetings, monthly peer review and supervised and mentored sessions.

Working in General Practice no two days are the same. The role is dynamic and exciting. We offer training and the opportunity to develop in areas of interest providing a supportive learning environment. We offer ongoing senior review, and technician support and the opportunity for individualised, structured project work.

Main duties of the job

You will be working as part of a wider practice and PCN team including GPs, nurse and paramedic prescribers, social prescribers, and physios. You will provide support to practice staff, both clinical and non-clinical, responding to queries, completion of medicines reconciliation and short- and long-term condition management. You will have a regular clinical commitment completing virtual, telephone and face to face reviews. You will be expected to support the teams medicines safety programme and dedication to improving overall patient care and safety, whilst observing the local and national medicines optimisation agenda.

Job responsibilities

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a multidisciplinary team in the PCN. In this role they will be supported by a senior clinical pharmacist who will support, develop, manage, and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

The candidate should have completed the 18month CPPE training programme Clinical Pharmacist in GP or be willing to undertake the training programme.

The post holder if not already a nonmedical prescriber will be supported to develop their role to become a nonmedical prescriber and development process once they qualify.
