Job summary

West4GPs is a well-established GP practice for over 110 years with approx. 9,000 patients. We have temporarily moved to Stamford Brook whilst a new health centre is being purpose built for the practice to return to in 2025.

We are a high QOF achiever and encourage continued professional development.

We are looking to recruit a Full time/Part time Practice Nurse who ideally has experience in areas such as childhood and travel immunisations, cytology, INR management, chronic disease management including, diabetes, asthma COPD etc.

The candidate should be able to work well in a team and demonstrate the ability to direct the work of the nursing team and be proactive in the development of the practice. They would also be expected to take clinical leadership in areas as required.

Main duties of the job

To provide nursing services to the whole practice population, including treatment, screening and advice within the professional competence of the post holder. This will cover chronic disease management and preventative care within the nurse remit.

The post holder should demonstrate critical thinking and skills in decision making in the management of patients. The expectation is that you will work collaboratively within the practice team to ensure that the needs of the patients are met and that delivery of policy and procedure are adhered to.

The post holder will be expected to work in accordance with the Professional Code of Conduct of the NMC. As an autonomous practitioner, you will be responsible for the care delivered.

About us

West4GPs is a friendly practice with a current list size of approximately 9,000 patients. As a practice we have been in situ for over 110 years.

We work with an amazing team of staff, both clinical and administrative. The atmosphere of the practice is supportive, caring and hardworking.

Currently we have 3 GP Partners, 3 Salaried GPs, a Nurse prescriber, Practice Nurses, 3 extremely competent Healthcare Assistants and a dedicated Nurse administrator working alongside 5 reception and 6 office staff.

The practice is part of Chiswick Primary Care Network and we have the benefit of a fabulous ARRS team consisting of Pharmacists, Physios, Paramedics, Social Prescribers and GP support administrators. We are an active member of the PCN.

Summary of Duties

To provide nursing services to the whole practice population, including treatment, screening and advice within the professional competence of the post holder. This will cover chronic disease management and preventative care within the nurse remit.

Key Responsibilities:

  • Accountability for own clinical work.
  • To provide nursing treatments to patients in participation with General Practitioners or independently, to agreed guidelines and protocols.
  • Providing nursing care and clinical services for a full range of procedures, such as wound care, vaccinations, immunisations, child immunisation clinics, INR clinic, blood pressures, phlebotomy, Asthma and COPD reviews and smears.
  • Managing caseloads
  • Developing and auditing standards of nursing care
  • Providing family planning advice
  • Assist doctors in examinations e.g. coil fittings/ removals
  • To be aware of, and act in accordance with, NMC standards and national policies, procedures and legislation.
  • To maintain the standards of professional behaviour of the Practice and other professional bodies

Preventative Health and Chronic Disease Management:

  • Managing and evaluating screening and immunisation programmes
  • Providing health screening, planned and ad hoc, to patients of all ages, and advising and taking action on the findings
  • In conjunction with nursing colleagues, managing the nursing care of all those patients with chronic disease and maintaining an appointment and recall system
  • To take an active role in nurse run clinics, e.g. asthma, diabetes, C.O.P.D, hypertension, and flu etc.
  • Assisting with the implementation, review and maintenance of infection control measures in the practice
  • Giving travel advice and vaccinations

Development, Teaching and Education

  • Participating in the continuing identification and assessment of the care needs of the practice population
  • In conjunction with the Personnel Manager, forming own personal development plan
  • Initiating/ contributing to the development and review of practice nurse protocols
  • To maintain professional development by attending courses and study days, as agreed with the general practitioners.
  • Maintaining accurate patient records both electronically and paper version
  • Maintaining equipment and supplies and ensuring correct storage of medical and surgical equipment, including vaccines
  • Summarising of patient records onto the clinical computer system
  • To participate in education and training of students of all disciplines, within the practice
  • Providing regular feedback to the Practice Manager, on request

Liaison and Team Communications:

  • Liaising with members of the wider primary care team and agencies outside the practice to ensure appropriate care is provided
  • Participating as an active member of the team, and attend meetings as agreed by the doctors/Practice Manager
  • All courses/training to be agreed by the doctors/practice manager prior to being booked

Care and Programme Management:

  • Initiate and carry out programmes of health screening
  • In line with practice policy, manage programmes of care for patients with acute and chronic diseases, by planning, providing and evaluating care
  • All the above points in line with surgery policies

Clinical Practice Development:

  • To enhance the nursing contribution and quality of care
  • Implement strategies for quality assurance and quality audit
  • Claim form administration
  • To participate in research within the practice

Nursing Management and Administration:

  • The team will be accountable for the nursing services. This includes equipment/ instruments/ disposables used in the practice nurses area, doctors rooms and in the surgery public areas, as well as vaccinations, drugs and all clinical stock used in the practice, including safe and appropriate storage.
  • To attend to patients who involuntarily emit bodily fluids e.g. vomit, blood, within the surgery premises, using appropriate cleaning materials, in line with health and safety recommendations.
