Job summary

We are a large forward thinking practice looking to recruit an experienced and enthusiastic Practice Nurse to join our growing team of multi-disciplinary clinicians, to deliver high quality care which meets the needs of our patient population whilst working within their professional boundaries.

Main duties of the job

We are ideally looking for a nurse with experience to provide and maintain a high standard of nursing care based on the needs of the Practice population which includes treatment, screening, preventative care, chronic disease management and patient education, as well as providing nursing assistance to the Doctors and other members of the primary healthcare team.

We would however be willing to provide training in all areas for the right applicant.

We pride ourselves in offering a friendly, efficient and professional service for approximately 25,000 patients operating over 3 sites, two sites are in Gorleston and one in Hopton.

You will work alongside our Practice Nurse Manger and a team of Practice Nurses, HCAs, Phlebotomists, supported by GPs, Advanced Nurse Practitioners, Paramedics, Pharmacists, Pharmacy Technicians, a Managerial team and administrative support staff.

Job responsibilities

Duties and Responsibilities

Clinical

To manage patients with chronic disease, running specific clinic sessions

Assess, plan, provide, implement, and evaluate nursing care to meet the care needs of individuals in the practice population.

Assess and care for patients with presenting wounds including leg ulcers and refer to leg ulcer clinic where appropriate according to leg ulcer protocol.

Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule

Provide travel health advice and administer appropriate vaccinations

Treat patients with minor injuries

Provide directly and assist GPs with contraceptive services

Act as chaperone when required

Undertake health promotion to help educate patients

Select, carry out and interpret results of specific tests

Follow infection control procedures as per infection control protocol and record as appropriate.

Administration

To maintain accurate patient records and enter onto the computer using agreed system templates and read codes

Ensure accurate completion of all necessary documentation associated with patient health care

To collect data and participate in regular Practice audit and research projects

To contribute to the preparation of any Practice development plans

General

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

To maintain a safe and therapeutic environment for all staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to

To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

Attend and participate in staff and Practice meetings and in-house clinical training sessions

Any other reasonable duties that may be required from time-to-time
