Job summary

Holmside Medical Group has an exciting opportunity for a Mental Health Link Worker to join our Mental Health Team. You would be joining an experienced team including a Senior Mental Health Link Worker and two Mental Health practitioners. Candidates with previous link worker experience and Mental Health training are preferred.

Main duties of the job

To work with partners & existing groups to develop the referral pathways for support. Draw on and increase the strengths and capacities of local communities, enabling local Voluntary Community and Social Enterprise organisations (VCSE) and community groups to receive social prescribing referrals. Keep up to date with organisations & feedback to team.

To work with individual clients, particularly around poor mental wellbeing & low level mental health issues,1:1 basis to build trusting relationships to ensure they are accessing the right support, time & place.

Provide personalised support to individuals, their families & carers to take control of their mental health & wellbeing, live independently & improve their health outcomes, as a key member of the PCN multi-disciplinary team. Develop trusting relationships by giving people time and focus on what matters to me. Possess genuine listening skills and provide short-term interventions such as CBT, and solution-focused techniques.

To manage and prioritise your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.

Alongside other members of the PCN multi-disciplinary team, work collaboratively with all local partners to contribute towards supporting the local VCSE organisations and community groups to become sustainable and that community assets are nurtured, through sharing intelligence regarding any gaps/problems identified in local provision with commissioners & local authorities

About us

Holmside Medical Group we are a split site practice with 10,000 patients in the West of Newcastle upon Tyne. We are in the West End Family Health Network, which includes Cruddas Park and West Road Medical Centre. We have collaborated together since 2014 prior to PCNs.

We have a diverse team of clinicians including GPs, Advanced Nurse Practitioner, First Contact Physio, Mental Health team, Nurses and Health care. We also have Health Coaches, Social Prescribers for adults and children and Care Coordinators. But above all we are friendly, well organised, innovative and really want to make a difference to our patients.

The main role of the Mental Wellbeing Social Prescribing Link Worker (SPLW) will be to work within the Primary Care Network (PCN) to support strengthening community and personal resilience and reducing health and wellbeing inequalities by addressing the wider determinants of health, particularly those impacting mental health / wellbeing, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities. The post holder will particularly work with people with poor mental wellbeing and low level mental health issues.

To encourage and facilitate the development and delivery of personalised care support plans with individuals particularly around poor mental wellbeing and low level mental health issues.

To apply asset based community development, engagement and consultation techniques as appropriate to engage the target groups/individuals.
