Job Summary
The Patient Navigator is responsible for managing
incoming eConsults and ensuring patients are directed to the most appropriate
clinician or service in a timely manner. The role supports efficient
appointment utilisation by reviewing, triaging (non-clinically), and allocating
eConsults to clinicians appointment lists, while also guiding patients toward
suitable alternative services for advice and care when appropriate.
Working hours will be Tuesday 12.30-17.00, Wednesday 12.30-17.00,Thursday 08.00-17.00 & Friday 08.00-12.00. Flexibility to cover leave and sickness is required.
Main duties of the job
Key Responsibilities
eConsult Management
- Monitor and process incoming eConsult submissions daily.
- Review eConsult information to determine the appropriate clinician or service based on established protocols.
- Allocate eConsults to relevant clinicians appointment lists in line with practice guidelines and capacity.
- Ensure urgent requests are escalated promptly according to practice procedures.
- Maintain accurate documentation within the clinical system.
Patient Navigation & Signposting
- Contact patients where clarification is required.
- Advise patients on the most appropriate service for their needs (e.g. GP, nurse practitioner, pharmacist, physiotherapist, mental health services, community services).
- Provide information on self-care options and external services (e.g. community pharmacy, NHS 111, local support services).
- Support patients in understanding how to access digital and in-person services.
Administrative & Coordination Duties
- Work closely with clinicians and reception teams to manage appointment capacity.
- Ensure appointments are booked appropriately and efficiently.
- Contribute to reducing avoidable GP appointments through effective signposting.
- Maintain patient confidentiality and adhere to data protection regulations.
- Participate in team meetings and service improvement initiatives.
