Job summary

Chet Valley Medical Practice is looking to recruit a full-time or two part-time paramedic practitioners.

The successful candidate will be a registered paramedic with post graduate qualifications who acts within their professional boundaries

Main duties of the job

The post-holder will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment (including prescribing of medication if qualified to do so) and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the practice, their own homes and care homes.

The paramedic practitioner will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

There is an expectation that training across the practice is maintained to an expected level with use of appropriate equipment and facilities as directed by the post holder who is an autonomous allied healthcare professional.

A supervision and mentorship network will be provided.

The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment.

The post holder must own their own vehicle as home visits will be an expectation.

Job description

Job responsibilities

The following are the core responsibilities of the Paramedic delivering health services.

a. Work as part of a multi-disciplinary team (MDT) within the practice.

b. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

c. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the social prescribing service and, where appropriate, other community or voluntary services

d. Be able to:

Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.

Perform and interpret ECGs.

Perform investigatory procedures as required and undertake the collection of pathological specimens including intravenous blood samples, swabs and other samples within their scope of practice and within line of local and national guidance.

e. Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients)

f. Provide an alternative model to urgent and same day GP home visit for the network.

g. Communicate at all levels across organisations ensuring that an effective, person-centered service is delivered.

h. Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required.

i. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation.

j. Communicate effectively with patients and, when appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices

k. Receive referrals and directed patients from triage services and other clinicians.

l. Maintain accurate clinical records in conjunction with extant legislation.

m. Develop yourself and the role through participation in clinical supervision, training and service redesign activities.

n. Actively signpost patients to the correct healthcare professional

o. Review the latest guidance, ensuring the organisation conforms to NICE, CQC etc.

p. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on safeguarding adults and safeguarding children.

q. Support in the delivery of enhanced services and other service requirements on behalf of the practice

r. Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events.

s. Undertake all mandatory training and induction programmes.

t. Contribute to and embrace the spectrum of clinical governance.

u. Attend a formal appraisal with your line manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

v. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care.

w. Maintain a clean, tidy, effective working area at all times.

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.

Essential

  • Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
  • Understanding of safeguarding adults and children
  • Commitment to reducing health inequalities and proactively working to reach people from all communities
  • Ability to work within own scope of practice and understanding when to refer to another healthcare professional
  • Ability to record accurate clinical notes

Desirable

  • Excellent interpersonal, influencing, negotiation and organisational skills with the ability to constructively challenge the views and practices of managers and clinicians
  • Good clinical system IT knowledge (SystmOne)

Qualifications

Essential

  • Registered with the Health and Care Professions Council (HCPC) as a Paramedic

Desirable

  • Educated to degree level in paramedicine or equivalent experience.
  • Working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement of reimbursement for that individual (or a longer time period as agreed with the commissioner)
  • Independent Prescribing Qualification
  • Is working towards developing Masters level or equivalent capability in paramedic areas of practice and has completed and been signed off formally within the clinical pillar competencies of the NHS E Paramedic FCP/AP Roadmap to practice
  • Completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.

Experience

Essential

  • Completed their two-year Consolidation of Learning period when a newly qualified paramedic
  • Experience of working in General Practice

Desirable

  • Completed a further three years experience as a band 6 (or equivalent) paramedic
  • Attained Level 7 capability in paramedic areas of practice
  • An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners
