The Holbrook and Shotley Practice are looking to appoint a highly motivated Paramedic with excellent clinical and communication skills to provide a first class minor illness and home visiting service for our 8000 patients on the Shotley Peninsular.

Main duties of the job
- work within the community, as an autonomous, accountable, PCN paramedic, in the provision of a holistic approach for individuals including assessment, management and treatment, to deliver high quality patient services, and to
- assess, manage, treat, refer and/or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

The Holbrook and Shotley GP Practice is part of South Rural PCN and this position is an ARRS role based solely within the Practice. South Rural PCN was formed in 2019, with five like minded practices wanting to provide a joined up service for local communities.To achieve this we are looking to reshape our clinical teams by developing new and exciting skill mixes, which not only diversify our workforce, but allows us to provide a better care package for our patients.
We pride ourselves on providing a high standard of care to our 8000 patients. Our practice team comprises of three GP Partners, two salaried GP's supported by a highly experienced team of nurses, dispensers, Clinical care practitioner, practice paramedic and reception team

Job responsibilities
- The
role will provide a specialist Paramedic resource for health care
professionals and service users, working in collaboration with other
members of the Multidisciplinary Team
- To work
within the community, as an autonomous, accountable, Paramedic
Practitioner. He/She will make
decisions and apply appropriate clinical interventions to the presenting
patient in their own home or in the surgery to deliver quality patient
services.
- He/she
will work autonomously but within the practice ‘s and HCPC policies,
protocols and guidelines
- To
assess, diagnose, treat, refer or signpost patients/service users
- The post
holder will use advanced clinical skills to provide education to service
users, promoting self-care and empower them to make informed choices about
their treatment
- The post
holder will have access to the on the day duty GP for advice and support
as necessary, for example patients needing referral or admission to secondary
care
- The post
holder must own vehicle for home visits with mileage expenses remunerated
by submission of a monthly mileage form. Please note it is the post
holder’s responsibility to ensure that their car insurance is covered for
business use
Clinical Practice
·
Work
within professional boundaries to accurately examine and assess patients with
an undifferentiated diagnosis, from a physiological, psychological and social
perspective, and plan clinical care accordingly on home visits/visiting care
homes.
·
Diagnose
and manage both acute and chronic conditions, integrating both drug- and
non-drug-based treatment methods into a management plan.
·
Prioritise
health problems and intervene appropriately to assist the patient in complex,
urgent or emergency situations, including initiation of effective emergency
care.
·
Work
with patients in order to support compliance with and adherence to prescribed treatments.
·
Ensure
full and accurate documentation in patient notes of clinical history, physical
examination, diagnosis and treatment plan.
·
Make
referrals as appropriate.
- Carry out Avoiding Unplanned Admission reviews.
- Assist with the annual flu immunisation
programme.

Ensure
clinical practice is safe and effective and remains within boundaries of
competence, and to acknowledge limitations.
- Works
from the surgery and within communities as an autonomous practitioner
caring for patients and families.
- Works as
an autonomous practitioner, in accordance with the Health Professions
Council. Ensure that personal and professional clinical standards are
maintained.
- To
undertake assessment for patients within the community using diagnostic
skills and initiation of investigations where appropriate.
- To
formally and informally impart knowledge and skills to colleagues
promoting peer review and best practice within the work environment.
