Clinical Pharmacist

Job responsibilities

The Clinical Pharmacist will support the Partners and other clinical staff in the surgery to deliver a programme of medicines optimisation and review, including managing hospital discharge medication changes, secondary care implemented medicines, CQC related medicine changes and practice medication queries. The post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation for the clinical staff within the Practice.

Qualifications

Essential

• - Degree in Pharmacy
• - Registration with General Pharmaceutical Council
• -Completed or willing to commit to completing CPPE pathway or other primary care training pathway

Desirable

• - independent prescribing qualification
• -excellent IT skills including experience in SystmOne
• -up to date knowledge of CQC regulations in general practice and in particular medicines management
• -knowledge and experience of working as a Clinical Pharmacist in General Practice

• -Proven ability to recognise and manage risk
• -Ability to work to personal limitations and when to refer to appropriate colleagues
• -Able to work flexibly with changing priorities
• -Strong problem solver

Desirable

• -Knowledge & experience of working as a Clinical Pharmacist in general practice
• -Knowledge of structured medication reviews
• -Frailty and Care Home experience
• -Participation in QIP and implemented change
