Job summary
We are looking to recruit a motivated, patient-focused Practice Nurse to join our friendly and supportive multidisciplinary team. This is an excellent opportunity for a nurse who is passionate about high-quality primary care and wants to work in a well-organised, forward-thinking GP practice.
The successful candidate will play a key role in delivering evidence-based nursing care, supporting long-term condition management, and contributing to preventative health services for our diverse patient population.
Main duties of the job
The Practice Nurse will be responsible for providing a wide range of clinical nursing services including:

Chronic disease management (e.g. diabetes, asthma, COPD, hypertension)

Immunisations and vaccinations

Cervical screening
-
Wound care and dressings
-
Health promotion and patient education
-
Supporting QOF and enhanced services delivery

Working collaboratively with GPs, HCAs, pharmacists and the wider PCN team
Carrying out relevant check such as fridge temprature checks, compliance with Infection Control procedures

Provide safe, effective, and patient-centred nursing care in a primary care setting

Undertake long-term condition management clinics including (but not limited to) diabetes, asthma, COPD, hypertension, and cardiovascular disease

Perform cervical screening in line with the NHS Cervical Screening Programme

Administer childhood and adult immunisations and vaccinations

Carry out wound care, dressings, and minor injury management

Support health promotion and disease prevention activities, including lifestyle advice and patient education

Maintain accurate, contemporaneous clinical records using EMIS Web
