Job summary
We are seeking an experienced and motivated Practice Triager (ANP or Paramedic Practitioner) to join our dynamic multidisciplinary team at Your Family Practice, providing same-day triage, assessment, and treatment for patients with acute presentations.
This is a key role within our patient access and clinical triage system, supporting safe, effective, and efficient care navigation for a list size of over 11,000 patients. Working alongside our GPs, Clinical Pharmacists, HCAs, and Triage assistans, you will play a vital role in managing patient demand, reducing GP pressure, and ensuring timely clinical decision-making.
Main duties of the job
Conduct same-day triage consultations (telephone, online, or face-to-face) to assess, diagnose and manage patient presentations in line with clinical guidelines and practice protocols.
Review all incoming patient requests via AccuRx, prioritising clinical urgency, risk, and safety.
Provide safe, evidence-based signposting to the most appropriate clinician or service, including self-care, pharmacy, first contact physiotherapy, mental health, or social prescribing, ensuring patients are navigated effectively through the system.
Manage minor illness and acute presentations, providing autonomous clinical assessment, diagnosis, treatment, and referral when required.
Support GP workload management by triaging, resolving, or redirecting appropriate cases independently, escalating complex cases to GPs when clinically necessary.
Contribute to proactive care by identifying vulnerable or high-risk patients during triage.
Document all triage decisions accurately on EMIS Web, maintaining clear safety netting and audit trails.
Contribute to service quality improvement, including triage safety audits, significant event reviews, and access performance reporting.
Engage in regular clinical meetings, supervision, and CPD to ensure ongoing professional development and best practice in primary care triage.

Clinical Triage and Assessment

Conduct same-day telephone, online, and face-to-face triage consultations to assess, diagnose and manage patient presentations in line with national and local protocols.

Review all incoming patient requests (AccuRx, eConsult, NHS App, and telephone) and prioritise based on clinical urgency, safety, and suitability for self-care, allied health, or GP review.

Provide autonomous assessment and management for a range of acute and minor illnesses within scope of practice.
