Clinical Pharmacist Croydon North Primary Care Network

Join our progressive and supportive team at Croydon North PCN, where youll play a key role in delivering high-quality, patient-centred care across Your Family Practice and Broughton Corner.

This is an exciting opportunity for a motivated Clinical Pharmacist to improve patient outcomes, enhance prescribing safety and reduce GP workload.

As an Independent Prescriber (desirable) or working towards; preference for cardiovascular) you will lead long-term-condition clinics, undertake Structured Medication Reviews (SMRs), reconcile hospital discharges and champion safe, evidence-based prescribing. Youll work closely with GPs, nurses and the pharmacy technician as part of an experienced multidisciplinary team.

Main duties of the job

As a key member of the Croydon North PCN clinical team, the Clinical Pharmacist will take a proactive role in improving patient outcomes through safe, effective and person-centred medicines management.

You will:

Conduct Structured Medication Reviews (SMRs) for patients with frailty, polypharmacy or high-risk medicines.

Lead diabetes and hypertension clinics, optimising therapy, lifestyle support and monitoring.

Reconcile medicines following hospital discharge, outpatient appointments or transfers of care, identifying and rectifying discrepancies.

Support implementation of MHRA alerts, NICE guidance and local formulary updates.

Deliver prescribing objectives linked to QOF and PCN DES.

Provide timely advice and medicines information to GPs, nurses, HCAs and administrative staff.

Participate in prescribing audits, clinical governance and quality-improvement initiatives.

Respond to medication related queries and urgent prescription requests to reduce avoidable GP appointments.

Promote self care, adherence and evidence-based, cost-effective prescribing.

  • Minimum 2 years post-registration experience in a patient-facing clinical role.
  • Sound understanding of medicines optimisation and clinical pharmacology.
  • Awareness of primary care workflows, repeat-prescribing and medication-safety systems.
  • Experience in medicines reconciliation, patient counselling and clinical queries.
  • Familiarity with NICE guidance, MHRA alerts and national prescribing frameworks.
