Clinical Pharmacist Croydon North Primary Care Network

As an Independent Prescriber (Desirable not essential, or working towards IP), you will lead diabetes and hypertension clinics, carry out structured medication reviews, reconcile hospital discharges, and champion safe, evidence-based prescribing. Youll be part of a dynamic multidisciplinary team, working alongside GPs, nurses, pharmacy technicians and social prescribers to improve access, outcomes and patient experience.

Main duties of the job
As a key member of the Croydon North PCN clinical team, the Clinical Pharmacist will take a proactive role in improving patient outcomes through safe, effective and person-centred medicines management. Youll manage your own clinics focusing on long-term conditions such as diabetes and hypertension, conduct structured medication reviews, and ensure safe prescribing across the network.
Youll work closely with GPs and the wider multidisciplinary team to reduce avoidable GP appointments, optimise treatment plans, and enhance prescribing quality. The role also involves supporting care homes, reconciling medicines after hospital discharge, and providing expert advice to both patients and colleagues.
Beyond direct patient care, youll contribute to local prescribing audits, quality improvement projects, and delivery of QOF and PCN DES priorities. This is a highly rewarding opportunity to lead on medicines optimisation and help shape safer, more efficient primary care across our practices.
