An exciting opportunity has arisen for a clinical pharmacist to join our established pharmacy team. North Croydon Primary Care Network are looking for an enthusiastic and highly motivated clinical pharmacist to join our forward thinking and patient focused team. The post holder is an experienced pharmacist, with a strong clinical background who acts within their professional boundaries. The successful candidate will be a key member of a multi-disciplinary team. They will operate in a patient-facing role, providing medication review services, delivering pharmaceutical care plans that maximize cost effective prescribing and improve the quality of patient care.
The role will suit a forward-thinking, innovative and dynamic clinical pharmacist who is able to work collaboratively across the team. The successful candidate will have regular supervision from a senior pharmacist and opportunities to develop their clinical practice. Main duties of the job
You should be able to provide professional advice and support on the safe prescribing, handling, and administration of medicines. This includes support regarding development and implementation of medicines management processes and policies. You will lead on the monitoring of high-risk medicines, reduce medicines-related risks and assist with medication reconciliation.
The successful candidate will proactively shape structured medication reviews, ensuring that medicines management processes within the practice are optimised to achieve highly effective, safe, and patient centred prescribing. As a PCN Pharmacist the post holder will work collaboratively within the team to help achieve the relevant requirements of the PCN DES. The PCN Pharmacist will lead the way in supporting patients, where necessary in taking their medicines to get the best from them (especially patients with medication issues and/or those with long term conditions), reducing waste, deprescribing where appropriate and promoting enhanced self-care.

The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities.
The successful candidate will provide support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation. The clinical pharmacist will provide expertise in clinical medicines advice to patients and members of the MDT in the GP practice.
The role will provide clinical leadership on medicines optimisation, quality improvement and be responsible for supporting the GPs in implementing effective medicine management, including doing searches and audits identifying areas for improvement, initiating and managing change.
The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The post holder must be prepared to undertake CPPE training for this role. If required, the post holder will also be supported to develop their role to become a non-medical prescriber.
