Job summary

We are looking for a motivated and enthusiastic practice nurse with a proven clinical background. The successful candidate will have the ability and personal drive to run clinics autonomously alongside Primary Care colleagues. You will possess excellent communication skills, a passion for patient care and the ability to adapt in order to respond to the challenging demands of primary care.

This is inititally a maternity cover position with potenitoal to become permanant for the right candiadate

Main duties of the job

To carry out a wide range of treatment room duties e.g. wound care, venepuncture, blood pressure checks, ECGs, injections and immunisations, leg ulcer management, chronic disease management, smears.

We are a small practice of 5500 patients with a family GP approach.

You will be working alongside our 3 GPs and healthcare assistant among a close knit friendly team.

Job Summary:

The post holder is responsible for the delivery of basic practice nursing services care to the practice population. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job Responsibilities:

Clinical

  • To carry out a wide range of treatment room duties e.g. ear syringing, wound care, venepuncture, blood pressure checks, ECGs, injections and immunisations, leg ulcer management, chronic disease management, smears
  • To advise patients on general health care and minor ailments with referral to the doctors as necessary
  • To participate in Nurse Triage where acceptable
  • To provide general and specific health screening/health promotion to the practice population, with referral to the doctors as necessary, in the areas of QoF management, Travel, Hypertension screening, Diet, Smoking cessation and other lifestyle factors, as agreed within practice protocols
  • To work with due regard for Health and Safety standards within the practice, including infection control procedures
  • To document care given and report back to the GP as appropriate
  • To participate in the cervical screening programme by raising patient awareness, competently carrying out cervical screening and ensuring results are delivered and acted upon appropriately
  • Implement and evaluate individual treatment plans for patients with a known long-term condition.

  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
  • Utilise communication skills to support patients to adhere to prescribed treatment regimens
  • Anticipate barriers to communication and take action to improve communication
  • To assist the Lead Nurse in maintaining effective communication systems with all practice team members, other agencies and the voluntary sector

Work Environment

  • The post holder is required to work frequently with bodily fluids
  • The post holder will regularly be required to carry out nursing care, which includes the moving and handling of patients with varying disabilities, and the handling of equipment
