Job summary

Do you have a passion for helping people make lasting, positive changes in their lives?

General Practice Alliance (GPA) are looking for a dynamic Health & Wellbeing Coach to deliver group-based support for working age adults with uncontrolled hypertension, as part of the Integrated Hypertension Support Programme.

This role is for 30 hours per week on a 13 month fixed-term contract.

We're looking for someone experienced, motivated and flexible, able to deliver structured group coaching both in person and online. The days you'll run sessions will be agreed in advance, with occasional evening or weekend work to meet participant needs.

Main duties of the job

As our Health & Wellbeing Coach, you'll deliver group support across GP practices, community venues and online, combining behaviour change coaching with practical tools such as isometric exercise, blood pressure self-monitoring, and the MyHeart app.

As part of an innovative, test-and-learn programme, you'll also play a key role in driving outcomes, gathering feedback and shaping continuous improvement.

Demonstrate and distribute Isobands, elastic resistance exercise bands used for strength training, rehabilitation, stretching, and mobility work. Supporting participants to safely complete isometric exercises as part of hypertension management.

Support participants to measure, understand and record their own blood pressure, promoting confidence and selfmanagement.

Enable signup and effective use of the MyHeart digital app, supporting users to track blood pressure, set goals and access educational content.

Encourage peer support and shared learning within group settings to build motivation and reduce health inequalities.

Collect and record lifestyle change data, blood pressure outcomes, attendance and engagement metrics.

Gather participant feedback and patient stories to support service learning, improvement and impact reporting.

Work collaboratively with GP practices, PCNs and community partners to promote the programme and maximise appropriate referrals.

  • Able to use a range of tools and techniques to enable and support people, including agenda setting, goal setting, and problem solving.
  • Ability to apply health coaching in group sessions, using active and reflective listening to build trust and rapport.

  • Ability to collect, interpret and record basic health metrics (e.g. blood pressure) with appropriate guidance and escalation.

Experience

  • Experience of delivering group-based interventions, workshops or coaching in health, community or wellbeing settings.
  • Experience of using coaching approaches, frameworks, models or other helping strategies (e.g. Motivational Interviewing)
  • Experience of supporting behaviour change in individuals or groups.

  • Experience delivering cardiovascular, lifestyle or prevention-focused programmes.
