Job summary

We are looking for an Enhanced General Practice Nurse to join our well-established nursing team at Pilning Surgery / Almondsbury Surgery.

We are looking to recruit an enthusiastic, approachable and knowledgeable nurse with an interest in chronic disease management / long term conditions. We are committed to embracing and encouraging talent.

The role will be based primarily at Pilning Surgery, working closely with the team at Almondsbury Surgery, and the Strategic Nurse Lead for Hanham Health, to deliver continued excellent leadership and governance.

You will have broad and excellent practice nursing skills, which include long term conditions, sexual health, wound care and ideally an independent prescribing qualification.

Salary will be according to experience.

Hours and working days are negotiable. We want to find the right fit for our existing team.

An enhanced DBS check will be required for this position.

Main duties of the job

Management of long-term conditions

Support compliance with, and adherence to, prescribed treatments

Wound care / ulcer care / removal of sutures

Health promotion and NHS health checks

Urinalysis

ECGs

Venepuncture to local guidelines

Blood pressure / hypertension reviews / Ischaemic Heart Disease reviews

Routine injections and immunisations / travel immunisations

Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management

Womens health checks including contraception, sexual health and cervical cytology

Support recall procedures, appropriate literature and health promotion, and audit

Adherence to infection control policies

Stock maintenance and management to include refrigeration, single use equipment and general treatment room equipment

Maximise Quality Outcomes Framework income

Experience of audit &and research

To take responsibility for own professional development and undertaking training for specific clinical tasks

Attend mandatory training courses and updates and any study days as appropriate to post

Participate in yearly appraisal, personal development plan and revalidation

Hanham Health operates across multiple sites in South Gloucestershire; Hanham Surgery, Oldland Surgery, Almondsbury Surgery (and from 1st June, Pilning Surgery), with a combined patient list size of 31,000.

We also hold the vasectomy contract for BNSSG.

The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care to the practice population.
