Are you an experienced practice nurse ready to take the next step?

We have an exciting opportunity for a General Practice Nurse to join our well-established nursing team at Almondsbury Surgery. We are looking to recruit an enthusiastic, approachable and knowledgeable nurse with an interest in managing a small team. Whilst previous experience of leadership is desirable, it is not essential as full training / support will be available for the right person to achieve this experience. We are committed to embracing and encouraging talent.

The role will be based primarily at Almondsbury Surgery, working closely with the Nurse Lead at Hanham Health, to deliver continued excellent leadership and governance.

Main duties of the job

You will have broad and excellent practice nursing skills, which include long term conditions, sexual health, wound care and ideally an independent prescribing qualification.

Salary will be according to experience.

Part-time hours (24), negotiable.

An enhanced DBS check will be required for this position.

Job Title: General Practice Nurse Lead

Accountable to: Employing General Practitioners

Role Summary:

To be responsible for the delivery of practice nursing services within a specified organisation, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.

To be responsible for a number of clinical areas such as health promotion, chronic disease management and health prevention.

To be responsible for developing, maintaining and reviewing clinical policy and procedure, including audit to ensure the organisation meets recognised standards.

Primary Responsibilities:

-Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health.

-Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.

-Provide vaccination and travel medicine services.

-Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.

-Maintain accurate clinical records in conjunction with extant legislation.

-Ensure read codes are used effectively.

-Maintain chronic disease registers.

-Develop, implement, and embed well woman clinics.

-Develop, implement, and embed well man clinics.

-Implement vaccination programmes for adults and children.

-Prioritise health issues and intervene appropriately.

-Recognise, assess, and refer patients presenting with mental health needs.

Secondary Responsibilities:

-Provide wound care (ulcer /Doppler etc.) to patients.

-Process pathology results as required.

-Implement and evaluate individual treatment plans for chronic disease patients.

-Chaperone patients where necessary.

-Support the team in dealing with clinical emergencies.

-Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.).

-Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).

-Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

-Act as the audit lead, effectively utilising the audit cycle.

-Monitor and ensure the safe storage, rotation, and disposal of medicines.

-Support junior members of the nursing team, providing guidance when necessary.

-Participate in local initiatives to enhance service delivery and patient care.
