We are seeking an enthusiastic and dynamic ANP to join our clinical team. We are looking for up to 30 hours per week.
We are a friendly, successful, 4 Partner practice situated in the heart of Thornbury committed to providing high quality patient health and care services to our c.9000 patients. Our ethos is built upon some core values including safe, accessible and continuous service for each of our patients.

Main duties of the job
We would expect the successful candidate to:
Work autonomously using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, diagnosed problems relating to minor illnesses or injury.
They should be autonomous in making decisions based on assessment, diagnosis and interpretation of test results.
They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. ANP's in this role will focus solely on same day medical need patients.
About us
We are a friendly, social team and fair well in the annual GP Patient Survey. We have a growing list size, hence looking to expand our workforce.
We are also a GP training practice with 3 of the 4 Partners registered as trainers, therefore we commonly have at least one learner with us at any given time. In addition, we are supported by 3 part time salaried GPs, 2 ANPs, a highly successful nursing team and a dedicated and supportive reception and administrative team.
We operate a more traditional model of general practice. We are in the early stages of introducing care navigation, most of our appointments are face to face and we strongly believe in continuity of care. Our GPs & ANPs work collectively as a team to ensure the well being of each other.

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure SNOMED codes are used effectively
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Extended and supplementary prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF etc.), complying with local and regional guidance
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
