Job summary
The role will involve
undertaking minor ailment clinics (primarily face-to-face appointments but
including telephone triage if necessary), with or without prior triage by
themselves or another clinician. The post-holder will perform chronic condition
reviews, including QOF reviews. They will undertake home visits and care home
reviews. It is desirable that the
successful candidate is a prescriber.

Main duties of the job
The clinician will assess,
diagnose, plan, implement and evaluate treatment/interventions and care for
patients presenting with an undifferentiated, undiagnosed problem. They will
diagnose and manage both acute and chronic conditions, with relevant clinical
support, and initiate and interpret and action tests/investigations as
appropriate. The clinician will preferably be a prescriber or in the process of
gaining a prescribing qualification.
Responsibilities include
reviewing and acting on documents/workflow as appropriate, working with
patients to support compliance with and adherence to prescribed treatments, and
prioritising health problems, intervening appropriately to assist patients in
complex, urgent or emergency situations, including the initiation of effective
emergency care.
The role also includes
supporting patients to adopt health promotion strategies that promote healthy
lifestyles and applying principles of self-care, as well as recording accurate
data in patient records to an agreed standard.
The post-holder will
collaborate with other members of the Primary Health Care Team including
doctors, nurses and other AHPs, accepting referrals and referring to them for
specialist care. They will be responsible for direct referral to and liaison
with consultants and other professional agencies.

- Masters level qualification in Advanced Clinical Practice or working towards it
- Independent Prescribing (V300)
- Evidence of CPD
- Current professional registration in good standing
