Job summary
We are looking for a proactive and experienced Pharmacy Technician to join our Primary Care Network (PCN) team, playing a pivotal role in delivering high-quality, patient-centred care. This is a varied and rewarding position that combines medicines optimisation, clinical support, and direct patient engagement to improve outcomes and reduce medicines-related risks for our patients.
Aspen is a large single-practice PCN; you'll work closely with pharmacy staff and prescription clerks, receiving direct support and supervision to thrive in your role.
As a key member of the multidisciplinary team, you will work closely with GPs, clinical pharmacists, nurses, and administrative staff to ensure patients receive safe, effective, and evidence-based treatment. You will be instrumental in supporting prescribing processes, resolving medication-related queries, and contributing to the development of safer, more efficient systems for repeat prescribing and medicines reconciliation.
A significant part of your role will involve running your own clinics, particularly for those with uncomplicated hypertension and hypercholesterolaemia, as well as other long-term conditions such as asthma, COPD, and hypothyroidism. These clinics will focus on medication reviews, monitoring requirements, and supporting patients to optimise their use of medicines. You will be empowered to make recommendations for therapy changes in line with clinical protocols and in collaboration with senior pharmacists and GPs.
Main duties of the job
- Provide telephone and face-to-face support to patients with questions or concerns about their medicines.
- Conduct clinical medication reviews, identifying potential interactions, safety concerns, or opportunities for deprescribing.
- Support the implementation of national and local prescribing guidance, including NICE recommendations and shared care protocols.
- Contribute to audit and quality improvement initiatives, helping to reduce medicines waste and improve prescribing safety.
- Assist in the management of medicines post-discharge, ensuring continuity of care and addressing discrepancies.
- Play a role in education and training, supporting practice staff in understanding medicines optimisation principles and safe prescribing practices.
This is a forward-thinking role with scope to develop and shape services, particularly in the area of long-term condition management. You will be encouraged to take initiative, contribute to service development, and work collaboratively across the PCN to deliver consistent, high-quality care.
