Job summary
We have an exciting opportunity for a clinical pharmacist to
join our existing clinical pharmacy team within Aspen Medical Practice and PCN
to support its development.
This role is pivotal to improving the quality of care and
operational efficiencies, so requires motivation and passion to deliver an
excellent service within general practice. It would be advantageous if you were
an independent prescriber within general practice, but it is not essential. As part of the role, if you
have not already undertaken this, you will be expected to complete the CPPE
primary care pathway which includes independent prescribing.

Main duties of the job
The clinical pharmacist will work as part of the general
practice team within their clinical competencies to provide expertise in
clinical medicines management and safety across a range of aspects such as
long-term condition management, day to day medication related queries, acute
prescription requests, supporting hospital discharge prescribing arrangements
and follow up, actioning MHRA alerts and targeted medication reviews. You will
have either face to face or telephone contact with patient. You will also be
involved in all aspects of leading and delivering on medicines safety. You will
be expected to help in streamlining processes and developing protocols to allow
for safer prescribing and to meet CQC expectations.
About us
Aspen Medical Practice was formed
in 2018 with the merger of four practices, making it the largest surgery in
Gloucestershire with over 30,000 patients, and the first single surgery Primary
care Network in the county. We work from
three sites, although the pharmacy team is wholly based at our largest site,
the Aspen Centre. We are a training
practice, with both under-and post-graduate students across a number of
clinical job roles, and we are a research active practice. Since our merger we have been developing our
teams to maximise the efficiencies that a large-scale organisation can bring to
its patients. We are building our
Clinical Pharmacy Team to ensure that we can meet the requirements of a large
and varied patient population. You will

- Masters degree in pharmacy (MPharm)
- Minimum of 2 years post-qualification experience.
Desirable
- Membership of the Royal Pharmaceutical Society

- Independent prescriber.
