Practice and memory services to ensure that our patients have a timely
diagnosis delivered compassionately and taking into account their personal
needs and circumstances. Ensuring that our patients physical health and care
needs are identified and met. Supporting patients with dementia, their
families, carers and our care homes.
Job summary
Alconbury & Brampton Surgeries are looking to recruit a
registered nurse (RN, RNMH, RNLD) with a passion for, and experience of,
providing relationship-centred dementia care with a strong sense of advocacy
and equality for our patients.
Main duties of the job
Main duties of the job
As our Dementia Nurse you will provide specialist one-to-one support, guidance, and practical solutions for families, alongside supporting best practice and education for our GPs and wider Practice Team.
Working with our lead GP to establish the clinical caseload of patients eligible for additional support, alongside setting-up, developing and evaluating the service.You will also influence and support the delivery of work streams that improve the experience of care for people living with dementia, and their families from pre-diagnosis to post-bereavement.
The successful applicant will be a NMC registered nurse, with significant post-registration clinical experience of working with people with dementia and supporting their carers/supporters in different settings, including the primary care setting, along with formal knowledge of dementia acquired through clinical post-graduate training/education.

You will be employed by Alconbury and Brampton Surgeries. You will work under the direct management of our GP Older Peoples Mental Health Lead.

About us
We are a 2 Surgery GP Practice with an older demographic and a new 70 bed care home with a residential care home with a Dementia floor assigned within.
We have 14000 registered patients. Our Practice clinical team consists of 5 GP Partners, 7 Salaried GPs, 2 Paramedics, 6 Nurses, 3 HCAs and a phlebotomist.
One of our GP Partners is the older Peoples mental Health Lead for the ICB and we have an HCA who is also developing an interest in caring for patients with Dementia. We work closely with the Alzheimers Society with regular support sessions for patients and carers. Brampton itself is accredited as a Dementia friendly village.
This post is intended to give us the opportunity to explore the extend of undiagnosed dementia within our Practice population and provide additional support to the team in caring for and supporting these patients and their families.
