Job summary

Are you enthusiastic about supporting people in accessing digital services and improving social inclusion? Are you a team player who is proactive and communicates effectively?

If so, we have an exciting opportunity to join our friendly and ever-expanding team as a Digital and Social Inclusion Coordinator, working between 3 PCN's across Bassetlaw.

This is a full-time position (37.5 hours per week) on a fixed-term contract for 12 months. You will play a key role in supporting patients to access digital healthcare services, improving confidence with technology, and helping reduce barriers to care within the community.

Main duties of the job

Working across Newgate PCN, Larwood PCN, and Retford & Villages PCN, you will help patients develop confidence in using digital health tools, promote inclusive access to primary care services, and support staff in maximising the benefits of NHS digital platforms. As part of our multidisciplinary team, you will champion digital inclusion and strengthen social connections for local people.

This post is expected to work between all 3 PCNs across Bassetlaw, so driving and access to a vehicle is essential. The post holder will work actively with all three PCNs but their main base and employer will be Newgate PCN.

The post holder will work between all 3 PCNs pro rata based on each PCNs patient ratio.

Newgate Medical Group (NMG) is a welcoming & busy single practice Primary Care Network (PCN) with over 30,000 patients. We have an established team of 9 GP Partners & a host of varied health professionals including pharmacists, nurses, ANP's, PA, Paramedics, Social Prescribing link workers, care co-ordinators & a large admin team.

Newgate is 1 of 3 PCNs within Bassetlaw and will host employment for this position. The candidate will work across all 3 PCNs in Bassetlaw which includes Larwood Health Partnership and Retford & Villages. The 3 PCNs have established collaborative working practices in integrated neighbourhood working and are forward thinking, supportive PCNs that embraces new ideas and innovation with the aim of producing better patient outcomes and efficient ways of working.

Our dedicated teams are crucial to the success of the PCNs and the service we provide to our patient population. Ongoing rigorous processes of evaluation and review inform every aspect of the services we provide, within each department, to ensure our high standards never slip. We aim to provide a good work life balance, support our staff to develop, feel engaged and part of the wider collaborative network.

This is an exciting time to join the team as we have plans to develop a new Health and Wellbeing Hub that will benefit residents and patients across the area whilst developing our team further.

Digital Access and Patient Empowerment

Actively promote and increase the use of digital health tools across the PCNs, such as the NHS App, online consultation platforms, and other digital communication systems.

Support patients in developing confidence and independence in managing their own health, care, and wellbeing through digital means.

Collaboration and Relationship Building

Build, maintain, and strengthen effective working relationships with colleagues, practices, and community partners across the three PCNs.

Help connect patients and services by developing strong networks that support the aims of digital and social inclusion.

Addressing Digital Barriers and Reducing Inequalities

Work collaboratively with stakeholders - including practice teams, patients, carers, and external agencies - to identify and reduce barriers to digital access, particularly for individuals experiencing significant health inequalities.

Support the development of inclusive approaches to ensure digital tools and services are accessible to all patient groups.

Project Coordination and Delivery

Coordinate and manage digital inclusion projects across the PCNs, ensuring delivery aligns with agreed objectives and timelines.

Facilitate workshops, task-and-finish groups, and other engagement activities; use effective communication and presentation skills to convey complex or sensitive issues to diverse audiences.

Monitor progress against project goals, evaluate outcomes, and provide regular reports to the PCN leadership team.

Use insights and data gathered through project work to inform, develop, and enhance digital services and patient access pathways.

Develop NHS App skills across Integrated Neighbourhood Teams and encourage NHS App Ambassadors sign-ups.

Practice Support and Service Improvement

Support GP practices in improving patient access to healthcare through digital tools, online consultation systems, and enhanced digital communication methods.

Assist practices in embedding digital solutions within service pathways to improve efficiency, consistency, and patient experience.

Governance, Compliance, and Professional Standards

Adhere to organisational policies, procedures, and mandatory training requirements to ensure safe, compliant, and effective working practices.

Contribute to a culture of quality improvement and reflective practice within the PCN teams.

Meetings and Representation

Attend relevant PCN, practice, and partner meetings as required, representing the digital inclusion function and contributing to collaborative planning and service development.
