Job summary
We
are looking for a Practice Nurse to provide high-quality, evidence-based care
to patients, particularly those with long-term conditions, including the
elderly and housebound. This role encompasses direct clinical care,
management of specialised clinics, and active participation in infection
prevention and control initiatives. The ideal candidate will demonstrate strong
clinical judgement, excellent communication skills, and a commitment to
continuous improvement.
Initially looking to recruit for a 6-month fixed
term but this may be extended.
Closing date set as 16th March, but may be extended until 30th March.
Main duties of the job
The Practice Nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, cervical screening, and immunisations.
You will manage the nursing care of our patients, ensure up to date and accurate clinical records are maintained and QoF reviews completed.
Your role will be varied and may include:
IPC, Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraceptions, Immunisations, Wound management, ECGs, Travel & other Vaccinations. You will be part of an existing team and work collaboratively with the wider team, including GP Partners, other clinicians, pharmacists, specialist and secondary care organisations.
You may be required to visit patients in their own home and support in our care home visits.

30,000 registered patients. We have an established team of 9 GP Partners &

PA & ANPs undertaking home visits & care home reviews. Also, social
prescriber link workers, Care Co-ordinators & excellent integration with

Primary Responsibilities
Assess
patient needs to ensure appropriate healthcare provision, incorporating evidence-based
practice
Develop,
implement, and evaluate individual treatment plans for chronic disease patients
to promote health and well-being
Identify,
manage, and support patients at risk of developing long-term conditions to
prevent adverse health effects
Provide
routine nursing care in accordance with clinical evidence, NICE, and NSF
guidelines
Diagnose
and manage acute and chronic conditions as required
Deliver
wound care (e.g., ulcer management, Doppler assessments) to patients
Review
patient medications to enhance compliance
Provide
guidance to patients on the use of prescribed and over-the-counter medications,
including side effects and dosages
Coordinate
travel medicine services
Request
and process pathology services and results as required
Oversee
chronic disease clinics, delivering patient care and referring to secondary or
specialist care as necessary
