Job summary

We are seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive team. The successful candidate will deliver high quality, evidence-based practice nurse care to our patient population and work collaboratively within the multidisciplinary team to support heath promotion, disease prevention, and long-term condition management.

Main duties of the job

Deliver a full range of practice nursing services in line with current guidelines, including wound management, venepuncture, Doppler studies, vaccinations (childhood, adult, travel), and injections via prescription or PGDs. Participate in national and local screening programs, including cervical cytology, and support long-term condition management with annual reviews, care planning, and patient education for diabetes, asthma, COPD, and cardiovascular disease. Apply infection control measures, manage emergencies (anaphylaxis, BLS), and support colleagues in safe practices. Provide advice on womens and mens health, family planning, and health promotion. Conduct audits, contribute to quality improvement, and mentor staff. Ensure safe storage, rotation, and disposal of drugs and vaccines. Work effectively within the practice team, manage resources efficiently, and maintain current evidence-based practice.

Key Responsibilities

Clinical Practice

- Deliver a full range of practice nursing services in line with current guidelines

- Wound management, including complex and non-complex wounds

- Venepuncture and specimen collection

- Undertake Doppler studies

- Health promotion and patient education

- Long term condition management with annual reviews

Immunisation & Vaccinations

- Administer vaccinations and immunisations including;

~ Childhood immunisations

~ Adult vaccinations

~ Travel immunisations

- Administer injections under Individuals Prescription or Patient Group (PGDs)

- Ensure safe storage, rotation, cold chain maintenance, and disposal of vaccines and drugs

National Screening Programmes

- Participate in national and local screening programmes, including:

~ Cervical cytology

Infection Prevention & Control

- Apply infection control measures in line with local and national guidance, including:

~ Universal hygiene precautions

~ Safe handling of specimens

~ Waste segregation and disposal

~ Decontamination of clinical equipment

~ Sharps injury reporting and management

~ Management of blood and bodily fluid spillages

- Support patients and colleagues in maintaining safe infection control practices

Emergency Care

- Demonstrate competence in emergency management, including:

~ Anaphylaxis

~ Basic Life support and resuscitation

Long Term Condition Management

Diabetes

- Basic understanding of diabetes management

- Support diagnosis and make appropriate referrals

- Conduct annual reviews

- Develop and support patient care plans

- Manage hypo/hyperglycaemic emergencies in line with practice guidelines

Respiratory Disease

- Basic understanding of asthma and COPD

- Conduct Asthma and COPD annual reviews

- Check inhaler technique and measure peak flow

- Provide patient education and self-management support

- Nebulise patients following assessment by a GP or senior nurse

Cardiovascular Disease

- Basic understanding of cardiovascular disease

- Identify and advise on cardiovascular risk factors (e.g BP, lipids, smoking, diet, exercise)

- Support management of hypertension, angina, myocardial infarction, and stroke patients.

Womens, Mens and & Family Health

- Provide advice on Womens Health, including cervical screening and breast self-awareness

- Provide basic advice on health promotion for Mens health issues

- Basic understanding of family planning and emergency contraception, with appropriate referral as required.

Leadership, Development & Teamworking

- Contribute to clinical audit and quality improvement activities

- Support and mentor colleagues, including unqualified staff where appropriate

- Work effectively as part of a wider practice team

- Contribute to the efficient use of nursing resources and stock control

- Maintain awareness of current evidence based practice and ethical considerations in general practice
