Job summary

West Mendip PCN is a progressive Primary Care Network that is looking to recruit a Pharmacy Technician to join us, supporting and working alongside a multi-disciplinary team.

The successful candidate will be based in a local cluster of General Practices as part of West Mendip Primary Care Network (WMPCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

Main duties of the job

The Digital Health Coach will support digital enablement work within the community, with a specific focus on addressing health inequalities. This work will include, but not be limited to, providing one-to-one and group support to patients registered at the five GP practices within the West Mendip Primary Care Network, relating to digital access and other projects.

West Mendip PCN is an NHS collaboration between the 5 practices of Street, Wells and Glastonbury. We have a population of 54,000 patients and are working together to provide additional and extended services to our patients.

Job description

Job responsibilities

Supporting and implementing community based digital work, which will include providing one to one and group support to patients. This will include working alongside other digital champion/connector roles within local communities and learning from peers within other local primary care networks

Regular attendance at Digital Cafes within local communities, working closely with Health Connectors who will refer patients for support with getting online, using healthcare apps and accessing free devices

Assist patients with access to their digital health record via the NHS App, whilst ensuring appropriate data protection measures are applied

Record data on our clinical system, ensuring coding is in line with national and local requirements, and ensuring digital work can be measured and evaluated

Working collaboratively with other members of team to develop new digital projects and ideas.

Develop links and partnership working with local and national partners such as businesses, councils, charities, and other individuals or groups

Take part in community engagement and awareness raising events with a focus on current digital projects and opportunities

Produce communications which promote and evaluate our digital enablement work in a format which is suitable for social media, newsletters and website content, and wider system sharing and learning

Maintaining professional and personal development

This post requires flexible working and scheduled working patterns to ensure service provision

Essential

  • 5 GCSEs at A-C or equivalent, including Maths and English

Desirable

  • Degree or other qualification in relevant area

Experience

  • Experience of working with people one-to-one and in groups to deliver training
  • Be able to evidence examples of data collection and evaluating projects and writing reports for different audiences
  • History of partnership or collaborative working and of building relationships across a variety of organisations

Desirable

  • Previous work with or understanding of the healthcare sector and Voluntary, Community and Social Enterprise (VCSE) sector

Knowledge & Skills

  • Good capability with software applications and aptitude to learn new systems quickly, and teach others
  • Excellent communication skills both verbal and non-verbal, 1:1 and with groups, and good written skills
  • Ability to organise, plan and prioritise on own initiative, including when under pressure, to meet deadlines and achieve targets

  • Ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way
  • Able to support people in a way that inspires trust and confidence, motivating others to reach their potential
