Job summary

Much Wenlock and Cressage Medical Practice is a member practice of Our Health Partnership, the largest super-partnership in the UK and are a member practice of the south-east Primary Care Network. We are looking to recruit an enthusiastic, dedicated Practice Nurse, with chronic disease management experience to complement our existing team.

Our clinical system is EMIS Web. We are high QOF achievers with a CQC Rating of Good.

The successful candidate will be motivated, a team player, and with an interest in improving and developing services under the leadership of the Management Team.

The post is for 3 days per week, 8.30 am - 6.00 pm. Provisional working pattern; Wednesday, Thursday, Friday.

Annual leave entitlement is 25 days pro rata (calculated in hours) plus statutory public holidays.

Pay based on AfC Band 5/6 scale 25/26* dependent upon experience.

Main duties of the job

The role will be varied and will include chronic disease management, care plans, treatment room work, national screening work such as cervical cytology and the national immunisation and vaccination schedule, providing travel health advice and administering appropriate vaccinations and health promotion to help educate patients.

Support: Nursing team, medical/nursing students

Job Purpose: To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education.

Duties and Responsibilities

Clinical

1.To manage patients with chronic disease and maintain robust call and recall systems

2.Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule

3.Provide travel health advice and administer appropriate vaccinations

4.Treat patients with minor injuries

5.Provide directly and assist GPs with contraceptive services

6.Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs

7.Act as chaperone when required

8.Undertake health promotion to help educate patients

9.Select, carry out and interpret results of specific tests

10.Undertake wound management

Administration

11.To maintain accurate patient records and enter onto the computer using agreed Snomed Codes and appropriate templates.

12.Ensure accurate completion of all necessary documentation associated with patient health care

13.To collect data and participate in regular Practice audit and research projects

14.To contribute to the preparation of any Practice development plans

General

15.To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

16.To maintain a safe and therapeutic environment for all Practice staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, CQC etc) are implemented and adhered to

17.To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

18.To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice
