Community Pharmacist and Clinical Pharmacist

Job summary

Your job will be a dual role, working as a community pharmacist in the partner owned Cromer Pharmacist, and as a clinical pharmacist within Cromer Group Practice. We are looking for someone who is highly motivated and wants to develop themselves as both as a community pharmacist and a practice based clinical pharmacist. Prior experience is valued but not essential.

You will have full access to surgery systems during all hours at work, enabling you to consider prescribing optimisation as part of your ongoing work. As a member of the practice team, you will be able to work closely with our clinicians to help improve patient care and make cost efficient prescribing decisions. Your role will be supported by out general practitioners who will help develop and mentor you.

In the pharmacy, you will be well supported, working alongside 4 ACTs and another pharmacist. The superintendent pharmacist and pharmacy manager ensure that the team is well run and supported day to day. Our ambition is to be offering as many pharmacy services are possible. We offer a range of NHS and private services.

Main duties of the job

Duties with the practice includes:

  • Conduct Structured Medication Reviews (SMRs) and post discharge medicines reconciliation.

  • Manage repeat prescribing, eRD, formulary adherence, and cost effective prescribing.

  • Run clinical sessions (e.g., hypertension treatment escalation, asthma/COPD optimization, polypharmacy, deprescribing).

  • Support QOF, IIF/PCN objectives and quality improvement projects/audits.

  • Provide clinical advice to GPs, nurses, HCAs and care coordinators; contribute to MDT huddles/meetings.

  • Handle medication queries from patients and care homes; deliver patient education.

  • Implement prescribing safety measures (e.g., high risk drugs monitoring, MHRA alerts).

  • Develop and update practice protocols/SOPs; contribute to clinical governance.

Cromer Group practice is a Coastal Practice with 12,500 patients. We have four GP partners three salaried GPs and a team of Nurse practitioners and Practice Nurses.

Qualifications

Essential

  • Masters degree in Pharmacy (MPharm)
  • Registration with the General Pharmaceutical Council (GPhC)
  • Independent Prescriber qualification (or willingness to undertake training)

Desirable

  • Experience in GP practice or primary care networks
  • Familiarity with SystmOne or similar clinical systems
  • Evidence of continuing professional development
